摘要
目的分析丁苯酞对老年急性脑梗死患者外周血Th17/Treg水平及相关细胞因子表达的影响,探讨其临床意义。方法将在我院接受治疗的97例老年急性脑梗死患者纳入研究,并将患者随机分为观察组和对照组,观察组50例,对照组47例。两组患者均给予常规治疗,观察组额外给予丁苯酞氯化钠注射液。于治疗前、治疗后7d及14d收集患者外周血标本,通过流式细胞术检测外周血Th17/Treg细胞水平,ELISA法检测血清IL-17、IL-6及TGF-β1水平。结果与治疗前相比,治疗7d及14d后,两组患者外周血Th17细胞水平降低,Treg细胞水平升高,Th17/Treg降低,之间差异均具有统计学意义(P<0.05),且7d后与14d后之间差异具有统计学意义(P<0.05)。治疗14d后,观察组外周血Th17细胞水平低、Treg细胞水平高、Th17/Treg低于对照组患者,之间差异均有统计学意义(P均<0.05)。与治疗前相比,治疗7d及14d后两组患者血清IL-17及IL-6水平逐渐降低,TGF-β1水平升高,差异均具有统计学意义(P<0.05),且治疗14d后IL-17及IL-6水平较7d时降低,TGF-β1水平升高,差异具有统计学意义(P<0.05)。治疗14d后,观察组血清IL-17及IL-6水平低于对照组,TGF-β1水平高于对照组,之间差异有统计学意义(P均<0.05)。结论丁苯酞能够对老年急性脑梗死患者发挥免疫调节作用,减轻患者机体炎症反应的强度,对脑组织发挥保护作用。
Objective To explore the influence and the clinical significance of butylphthalide on Th17/Treg cells and relative cytokines in the serum of elderly patients with acute cerebral infarction. Methods 97 patients with acute cerebral infarction,who were diagnosed and treated in our hospital from Jan 2016 to Jan 2017,were enrolled in this study and were divided into the observation(50 case) and control group(47 case).Both of the two groups were treated with regular treatments,and butylphthalide was given to the observation group.Blood samples of all patients were collected before treatment or 7 and 14 days after treatment.Peripheral Th17/Treg cells were measured by flow cytometry assay,and serum IL-17,IL-6 and TGF-β1 were tested by ELISA assay. Results 7 and 14 days after treatment,peripheral Th17 cells were significantly decreased and Treg cells were significantly increased( P <0.05),and there were significant difference between 7 and 14 days for Th17 and Treg cells in both groups( P <0.05).The ratio of Th17/Treg at 7 and 14 days after treatment were significantly lower than that before treatment,the ratio at 14 days was significantly lower than that at 7 days( P <0.05).14 days after treatment,peripheral Th17 of the observation group was significantly lower than that of control group,while Treg was significantly higher ( P <0.05).14 days after treatment,the ratio of Th17/Treg of the observation group was lower than that of the control group( P <0.05).The serum levels of IL-17 and IL-6 of both groups were significantly decreased,and the TGF-β1 level was significantly increased after treatment( P <0.05),and the levels of IL-17 and IL-6 at day 14 after treatment were significantly lower than that at day 7,while the TGF-β1 level was significantly higher( P <0.05).14 days after treatment,the serum levels of IL-17 and IL-6 of the observation group were significantly lower than that of the control group,while the TGF-β1 of the observation group were significantly higher than that of the control group( P <0.05). Conclusion Butylphthalide can regulate the immune function of elderly patients with acute cerebral infarction,alleviate the intensity of inflammatory reaction and protect the brain tissue.
