右美托咪定对复合麻醉患者血流动力学及神经认知功能的影响 被引量:7

Study on the protective effect of dexmedetomidin on hemodynamics and neurocognitive function in patients with compound anesthesia

摘要 目的 探讨右美托咪定对复合麻醉患者血流动力学以及神经认知功能的影响。方法 选取2014年9月至2016年8月在青海省人民医院行复合麻醉的患者105例,采用随机数表分为实验组(n=54)和对照组(n=51)。对照组给予生理盐水泵入,实验组给予右美托咪定干预。比较治疗前后两组患者收缩压、舒张压、心率、简易智力状态量表(mini-mental state examination,MMSE)、视觉模拟疼痛评分(visual analogue scale,VAS)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)、白介素-1β(interleukin-1β,IL-1β)、白介素-6(interleukin-6,IL-6)、血清神经生长因子(nerve growth factor,NGF)、脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)、血清核转录因子(nuclear transcription factor-κB,NF-κB)变化,并比较两组患者满意度。结果 治疗后,两组患者收缩压、舒张压、心率等指标均有显著降低,实验组以上指标明显低于对照组(P<0.05);治疗后,两组患者MMSE评分均显著降低,但实验组高于对照组(P<0.05);治疗后,两组患者VAS评分均显著降低,且实验组明显低于对照组(P<0.05)。治疗后,两组患者炎性因子表达水平均显著降低,且实验组明显低于对照组(P<0.05);治疗后,两组患者NGF、BDNF、NF-κB等指标均降低,但实验组明显高于对照组(P<0.05)。实验组患者的满意率为96.29 %,明显高于对照组的80.39 %(P<0.05)。结论 复合麻醉患者给予右美托咪定干预,可改变血流动力学指标,降低炎性因子表达水平,对神经认知功能具有一定保护作用。 Objective To investigate the effect of dexmedetomidin on hemodynamics and neurocognitive function in patients with compound anesthesia. Method 105 cases from September 2014 to August 2016 in our hospital underwent anesthesia, divided into the experimental group using a random number table method (n=54) and control group (n=51). The control group was pumped into the saline, and the experimental group was treated with dexmedetomidine. Compared before and after treatment in two groups of patients with systolic blood pressure and diastolic blood pressure, heart rate, Mini Mental State Scale (MMSE score), visual analogue score (VAS score), tumor necrosis factor alpha (TNF-alpha), interleukin -1 beta (IL-1 beta) and interleukin -6 (IL-6), serum nerve growth factor (NGF), brain-derived neurotrophic factor (BDNF), serum nuclear transcription factor (NF-κB) changes, comparison of patient satisfaction rate. Result After treatment, two groups of patients with systolic blood pressure and diastolic blood pressure, heart rate and other indicators were lower in experimental group, the above indexes were significantly lower than the control group (P<0.05);after treatment, the MMSE scores of two groups were decreased, but the experimental group was higher than control group (P<0.05);after treatment, the VAS scores of two groups were reduced, experiment group was significantly lower than the control group (P<0.05). After the treatment, the expression level of two groups of patients with inflammatory factors decreased, but the above index of experimental group was significantly lower than the control group (P<0.05);after treatment, two groups of patients with NGF, BDNF, NF-κB and other indicators were lower, but the above index of experimental group was significantly higher than the control group (P<0.05);after treatment, the patients in the experimental group satisfaction rate 96.29% 80.39% higher than the control group (P<0.05). Conclusion The intervention of dexmedetomidine in compound anesthesia can improve hemodynamics and reduce the expression level of inflammatory factors, and has a protective effect on neurocognitive function.
