摘要
1例56岁男性患者,因左下肢无力伴左侧口角及左手麻木感3d入院。既往糖尿病病史8年,平素口服瑞格列奈片降血糖,否认药品不良反应史。入院后予抗血小板、调血脂、改善循环等治疗,继续口服瑞格列奈降糖。入院第5日,患者于夜间出现心悸伴出汗,自服食物后好转,入院第6日夜间再次出现类似反应,急查血糖示3.7mmol·L-1,临床药师考虑CYP2C8潜在抑制剂氯吡格雷和瑞格列奈相互作用致低血糖可能性大,建议医师停用瑞格列奈,继续监测血糖,未再发生低血糖反应。
One 56-year-old male patient was admitted to the hospital due to weakness of left lower limb and numbness of left hand and the left side of the mouth for 3 days. The patient had 8 years history of diabetes controlled by oral repaglinide and denied history of adverse drug reactions. After admission, drugs including repaglinide were given to him for anti-platelet therapy, regulating lipid, improving circulation and lowering glucose. On the night of the fifth day, the patient developed palpitations with sweating, which was improved after taking food. The similar symptoms appeared at the night of the sixth day. Emergency lab test showed that the blood glucose was 3.7 mmol·L-1. Considered that hypoglycemia was caused by the drug interaction between the potential inhibitor of CYP2C8 (clopidogrel) and repaglinide, clinical pharmacist suggested stopping repaglinide, monitoring blood glucose and there were no hypoglycemia after withdrawl of repaglinide.
