摘要
目的:回顾性分析及探讨磁共振胰胆管成像(MRCP)联合运用磁共振弥散加权成像(DWI)对婴幼儿胆道闭锁的诊断价值。方法:收集上海市儿童医院2014年7月至2017年11月共42例经手术和病理证实为胆道闭锁患儿的临床资料,所有患儿术前均接受DWI、3D-MRCP和常规MRI检查,重点分析胆道闭锁的3D-MRCP和DWI影像表现,并对比联合运用DWI前后对胆道闭锁的诊断符合率。结果:3D-MRCP对胆道闭锁能够准确定位,结合DWI后能提高对该疾病的定性诊断。联合运用DWI后诊断符合率从73%提高至90%,两种方法的检査结果差异具有统计学意义(PV0.05)。结论:3D-MRCP联合DWI后对Ⅲ型胆道闭锁的诊断更直观准确,具有一定的意义。但对于Ⅰ型及Ⅱ型胆道闭锁的分型诊断意义不大。
Purpose: To retrospectively analyze and discuss the diagnostic value of magnetic resonance cholangiopancreatography (MRCP) combined with magnetic resonance diffusion weighted imaging (DWI) for infant biliary atresia. Methods: The clinical data of 42 cases with biliary atresia confirmed by operation and pathology results were collected in Shanghai Children's Hospital from July 2014 to November 2017. All the children were examined by DWI, 3D-MRCP and routine MRJ before operation. The MRCP and DWI images of biliary atresia were analyzed. The diagnostic accuracy of biliary atresia was compared before and after the combined use of DWI. Results: The location of the biliary atresia could be accurately displayed by 3D-MRCP, and the diagnostic accuracy of the disease could be improved by combining with DWI. The diagnostic coincidence rate of the 3D-MRCP combined with DWI increased from 73% to 90%, and the differences in the diagnostic results of the two methods were with statistical significant (P<0.05). Conclusion: The diagnosis of type-Ⅲ biliary atresia by 3D-MRCP combined with DWI is more accurate, and it is of certain significance. However, the diagnosis of type Ⅰ and Ⅱ biliary atresia is of little significance.
