摘要
目的:探讨手法治疗联合冲击波治疗髌尖末端病的临床疗效。方法:选取2016年1月-2017年1月我院收治的90例髌尖末端病患者作为研究对象。患者均为单侧发病,长时间行走后打软腿、上下楼痛、蹲起痛,检查有髌尖压痛、部分人有股四头肌萎缩和伸膝抗组痛,结合影像学检查确诊为髌尖末端病,采用随机数字表法将其分为A、B、C组,A组30例单纯应用手法治疗,医生首先用手固定髌骨上缘,挤压髌上囊部位向下压迫髌骨上缘,使髌骨下缘向上翘起,对髌尖疼痛处采用点法和刮法治疗,B组30例单纯应用冲击波治疗,应用弹道式冲击波治疗仪治疗,以患者髌尖压痛最显著的地方作为中心开始治疗,合理选择冲击波治疗参数,C组30例采用手法治疗联合冲击波治疗,比较三组髌尖末端病的治疗效果。结果:A组髌尖末端病患者治疗后总有效率为63.33%,B组髌尖末端病治疗总有效率为66.67%,C组髌尖末端病治疗总有效率为96.67%,A组髌尖末端病治疗总有效率与B组比较无显著差异(P> 0.05);但C组治疗总有效率明显高于A、B组,P <0.05,具有统计学意义。结论:采用手法推拿联合冲击波治疗髌尖末端病的临床疗效较为理想,能够有效改善髌尖末端病患者局部肿胀、疼痛的症状,促进患者膝关节功能的恢复,并且手法推拿结合冲击波治疗方案患者易于接受,在临床各级医院均可实施,具有临床应用及推广价值。
Objective: To explore the clinical efficacy of manipulation combined with shock wave in the treatment of patellar apical terminal disease. Methods: 90 patients with patellar apical terminal disease admitted to our hospital from January 2016 to January 2017 were selected as study subjects and were divided into groups A, B and C by random number table method, 30 cases in group A were treated with manipulation alone, 30 cases in group B were treated with shock wave alone, and 30 cases in group C were treated with manipulation combined with shock wave, and the therapeutic effects of the three groups of patellotella apical terminal disease were compared. Results:There was no significant difference between group A and group B in total effective rate(P>0.05). However, the total therapeutic efficiency of group C was significantly higher than that of group A and group B, P<0.05, showing statistical significance. Conclusion: Manipulation massage combined with shock wave was effective in the treatment of patellar apical terminal disease, it could effectively improve local pain and swelling symptoms, and had positive effects on the recovery of joint function.
