术中保温对胃肠开腹手术患者低体温及手术部位感染的影响 被引量:72

Effect of intraoperative waring on hypothermia and surgical site infection in patients undergoing gastrointestinal open surgery

摘要 目的探究术中保温对胃肠开腹手术患者低体温及手术部位感染(SSI)的影响。方法选取2016年2月-2017年6月经医院行胃肠开腹手术患者100例,采用随机数表法,分为对照组(n=50)和研究组(n=50)。对照组给予盖被保温,研究组在对照组基础上给予医用升温毯保温,观察两组失血量、输血量、液体输入量、手术时间和患者术前,术中30 min、60 min、90 min,手术结束时的体温及凝血指标变化,比较两组患者低体温及SSI发生率。结果研究组失血量及输血量显著低于对照组(P<0.05);研究组术后血红蛋白(Hb)显著高于对照组(P<0.05);术中及手术结束时,研究组体温均显著高于对照组,凝血指标均显著低于对照组(P<0.05);术中,研究组收缩压(SBP)、舒张压(DBP)及血糖水平均显著低于对照组(P<0.05);研究组SSI发生率为4.00%(2/50)较对照组14.00%(7/50)低,差异具有统计学意义(P<0.05);研究组患者低体温发生率为8.00%(4/50)较对照组66.00%(33/50)低,差异具有统计学意义(P<0.05)。结论胃肠开腹手术患者术中应用医用升温毯,可有效改善术中应激反应,显著降低围手术期低体温及SSI的发生。 OBJECTIVE To explore the effect of intraoperative warming on hypothermia and surgical site infection(SSI) in patients undergoing gastrointestinal open surgery. METHODS A total of 100 patients who underwent the gastrointestinal open surgery in the hospital from Feb 2016 to Jun 2017 were enrolled in the study and randomly divided into the control group with 50 cases and the study group with 50 cases. The control group was given heat preservation by covering quilts, while the study group was given heat preservation by using medical warming blanket on basis of the treatment of the control group. The blood loss volume, blood transfusion volume, fluid input volume and operation duration of the two groups of patients were observed, the body temperature and blood coagulation indexes were observed before the surgery, at 30, 60,90 min during the surgery and at the end of surgery, and the incidence rates of hypothermia and SSI were compared between the two groups of patients. RESULTS The blood loss volume and blood transfusion volume of the study group were significantly less than those of the control group(P<0.05);the hemoglobulin(Hb) level of the study group was significantly higher than that of the control group after the surgery(P<0.05). The body temperature of the study group was significantly higher than that of the control group during the surgery and at the end of surgery, and the blood coagulation indexes of the study group were significantly lower than those of the control group(P<0.05). The systolic blood pressure(SBP), diastolic blood pressure(DBP) and blood glucose level of the study group were significantly lower than those of the control group during the surgery(P<0.05). The incidence rate of SSI of the study group was 4.00%(2/50),significantly lower than 14.00%(7/50) of the control group(P<0.05). The incidence rate of hypothermia of the study group was 8.00%(4/50),significantly lower than 66.00%(33/50) of the control group(P<0.05). CONCLUSION The intraoperative use of medical heating blanket may effectively improve the intraoperative stress response of the patients undergoing gastrointestinal open surgery and remarkably reduce the incidence rates of hypothermia and SSI during the perioperative period.
