摘要
目的建立3种伴有不同程度骨吸收的下颌骨囊性病变数字化模型,对比分析手术治疗后下颌骨的生物力学变化。方法构建轻、中、重度3种不同程度骨吸收的单侧下颌第三磨牙区囊性病变模型,模拟的骨缺损范围包含经牙槽嵴顶入路手术治疗带来的骨缺损。比较相同应力条件下的下颌骨生物力学特点及强度变化。结果轻度及中度颌骨吸收模型上常规咀嚼力量载荷并未引起下颌骨的力学强度的明显改变,而重度颌骨吸收模型在咀嚼力量载荷下则表现出了外斜线区的应力集中,接近骨性结构的屈服强度。结论生物力学分析提示颌骨囊性病变引起大部分皮质骨吸收后可导致下颌骨生物力学强度显著下降,是选择开窗减压并行二期手术治疗的指征之一。
Objective To establish three digital models of mandibular cystic lesions with different degrees of bone resorption, then, comparing and analyzing the biomechanical changes of mandible after operation. Methods Three mandibular models of unilateral cystic lesions with different degrees of bone resorption were constructed. The bone defects caused by surgical treatment via alveolar crest approach were also simulated in all models. The biomechanical characteristics and strength changes of mandible under the same restriction and force conditions were compared. Results Conventional masticatory force load on mild and moderate mandibular absorption models did not cause significant changes in mechanical strength of mandible, while severe mandibular absorption models showed stress concentration in the external oblique line zone under masticatory force load, which was close to the yield strength of bone. Conclusion Biomechanical analysis indicates that most of the cortical bone resorption caused by cystic lesions of the jaw can significantly reduce the biomechanical strength of the mandible, which is one of the indications for decompression and secondary surgical treatment.
