武汉市2009—2018年输入性非恶性疟病例血液检验指标 被引量:1

Blood test index in imported non-falciparum malaria cases in Wuhan, 2009-2018

摘要 目的探讨输入性非恶性疟的流行特征及血液临床检验指标特征,为诊断及预防提供科学依据。方法收集2009-2018年武汉市境外输入性间日疟、卵形疟和三日疟的非恶性疟确诊病例的流行病学资料和生化检验结果,分析病例发病-诊断时间与血液指标的相关性。结果共收集28例完整的流行病学和临床血液检测资料,其中间日疟9例、卵形疟15例、三日疟4例,均为境外输入性非恶性疟。间日疟主要由亚洲和东北非洲输入,卵形疟和三日疟全部由撒哈拉以南非洲输入。12例回国后超过3个月发病,潜伏期最长14个月,发病-诊断时间1~30 d,超过6 d 16例,中位数7.50 d。与血液指标正常值比较,28例非恶性疟血红蛋白、白蛋白下降,总胆红素、间接胆红素升高,肾功能正常。发病-诊断时间超过6 d组的病例红细胞、白细胞计数、血红蛋白及红细胞压积较6 d内组下降(P<0.05)及淋巴细胞比率恢复正常值水平,差异有统计学意义(P<0.05)。结论境外输入性非恶性疟感染致贫血和肾损害不明显,长潜伏期和发病-诊断时间较长者,血红蛋白、红细胞压积、血小板和总蛋白及白蛋白下降明显,三日疟贫血较明显。 Objective To explore the epidemiological characteristics of imported vivax malaria,oval malaria and malariae malaria and the characteristics of blood clinical test indicators,so as to provide scientific basis for diagnosis and prevention.Methods The epidemiological data of imported cases of vivax malaria,oval malaria and malariae malaria and the first blood routine and biochemical tests resultsin Wuhan from 2009 to 2018 were collected.The epidemiological characteristics and blood test indicators were studied by descriptive analysis,and the correlation between different plasmodium,"attack-diagnosis" time and blood test indicators were analyzed by statistical methods.Results A total of 28 complete epidemiological data and clinical blood test results were collected,including 9 cases of vivax malaria,15 cases of oval malaria and 4 cases of malariae malaria which vivax malaria was mainly imported from Asia and Northeast Africa,while both oval malaria and malariae malaria were all imported from sub-Saharan Africa.12 cases had onset after 3 months later returning to China and the longest incubation period was 14 months,attack-diagnosis interval was from 1 to 30 days,16 cases was more than 6 days and the median was 7.50 days.Compared with the normal blood index,the HGB and ALB of 28 cases of non-falciparum malaria decreased,TBIL and IBIL increased,and renal function was normal.The BBC,WBC.,HGB and HCT of patients with "After 6 days of attack-diagnosis(>6 days)" team were lower than those of patients "Less than 6 days of attack-diagnosis(≤6 days)" team(P<0.05)and The LYM% returned to the normal level,the difference was statistically significant(P<0.05).Conclusion The imported non-falciparum malaria ases didn’t lead to anemia and kidney damage.However,because of the long incubation period and the long attack-diagnosis time,HGB,HCT,PLTTP and ALB decreased,Liver function damage of oval malaria was more obvious,while anemia of malariae malaria was more obvious.
