摘要
高血压是临床最常见的慢性非传染性疾病,高血压的防治和管理是基层医务人员的重要工作内容之一。面对高血压高患病率的现实状况,国内外均在以高血压防治指南为基础的社区高血压管理实践中,探索了很多有效的社区高血压管理模式并各有特点。全文论述国内外社区高血压管理的主要模式及特点,为进一步规范我国的社区高血压管理实践提供相关借鉴。
Hypertension is the most common chronic non-communicable disease in clinic, the prevention and management of hypertension is one of the important tasks of the primary medical staff. Facing the reality of high prevalence of hypertension, many effective community hypertension management modes have been developed in the community hypertension management practice based on the guidelines of prevention and treatment of the hypertension at home and abroad. And each mode has its own characteristics. This paper summarizes the main modes and characteristics of community hypertension management at home and abroad in order to provide relevant reference for further standardizing the practice of community hypertension management in China.
