孟鲁司特钠联合布地奈德吸入治疗小儿哮喘的效果及对气道功能、血清炎性因子的影响 被引量:27

Effect of Inhaling Montelukast Combined with Budesonide on Asthma in Children and Its Effects on Airway Function and Serum Inflammatory Factors

摘要 目的探讨孟鲁司特钠联合布地奈德吸入治疗小儿哮喘的临床效果及对患儿气道功能、血清炎性因子的影响。方法选取2017年5月—2018年6月我院收治的160例哮喘患儿,采用随机数字表法分为研究组和对照组各80例。在常规治疗基础上,对照组给予布地奈德吸入治疗,研究组给予孟鲁司特钠联合布地奈德吸入治疗,均连续使用4周。治疗后比较两组疗效,治疗前后气道功能指标[呼气流速(TEF)25%、TEF50%、TEF75%、第1秒用力呼气容积(FEV1)/用力肺活量(FVC)]及血清炎性因子[白细胞介素4(IL-4)、IL-6、IL-8及肿瘤坏死因子α(TNF-α)]变化。结果研究组总有效率为92.5%高于对照组的77.5%(P<0.01)。治疗后研究组TEF25%、TEF50%、TEF75%及FEV1/FVC水平均较治疗前改善,且改善程度优于对照组,差异均有统计学意义(P<0.05或P<0.01)。治疗后两组IL-4、IL-6、IL-8及TNF-α水平明显降低,且均低于对照组治疗后,差异均有统计学意义(P<0.05或P<0.01)。结论孟鲁司特钠联合布地奈德吸入治疗小儿哮喘可提高临床效果,改善患儿气道功能,降低机体炎性反应程度,有利于患儿康复。 ObjectiveTo investigate the effect of inhaling montelukast combined with budesonide on airway function and serum inflammatory factors in children with asthma.MethodsA total of 160 children with asthma treated in our hospital from May 2017 to June 2018 were divided into the research group ( n =80) and the control group ( n =80) according to random number table method.The control group was treated with budesonide inhalation on the basis of conventional treatment, and the research group was treated with montelukast combined with budesonide on the basis of the control group.They were treated for four weeks.The efficacy of the two groups was compared after treatment.The airway function indexes [ tidal expiratory flow (TEF) 25%, TEF 50%, TEF 75%, forced expiratory volume in 1 second (FEV 1)/ forced vital capacity (FVC)] and serum inflammatory Factors [interleukin 4 (IL-4), IL-6, IL-8, and tumor necrosis factor alpha (TNF-α)] before and after treatment were compared.ResultsThe total effective rate of the research group was 92.5%, which was higher than that (77.5%) of the control group ( P <0.01).After treatment, the TEF25%, TEF50%, TEF75% and FEV 1/FVC levels in the research group were better than those before treatment, and the improvement was better than the control group( P <0.05 or P <0.01).The levels of IL-4, IL-6, IL-8 and TNF-α in the two groups were significantly lower after treatment( P <0.05 or P <0.01).ConclusionIn the treatment of asthma in children, inhaling montelukast combined with budesonide can improve clinical efficacy and airway function of patients and reduce the inflammatory response of body, which is favorable for children's recovery.
