摘要
目的对肺癌患者肺部感染的病原菌分布特点和耐药情况进行分析,为临床合理用药提供依据。方法回顾性分析2015年9月-2017年8月杭州市师范大学附属医院392例肺癌合并肺部感染住院患者临床资料,对分离的417例痰液进行病原菌鉴定和药敏试验,并对病原菌分布及耐药性进行统计分析。结果417例患者痰液标本中共检出病原菌133株,感染率为31.89%;其中革兰阴性菌87株,占65.41%,以鲍氏不动杆菌、铜绿假单胞菌、大肠埃希菌为主,分别占24.06%、16.54%、9.02%;革兰阳性菌36株,占27.07%,以金黄色葡萄球菌和肠球菌为主,分别占15.79%和11.28%;真菌10株,占7.52%。药敏结果显示,革兰阴性菌对亚胺培南、阿米沙星敏感,革兰阳性菌对万古霉素和利奈唑胺高度敏感。结论肺癌并发肺部感染患者的病原菌以革兰阴性菌为主,选择合理抗菌药物,可降低病原菌的耐药率。
Objective To analyze the pathogenic bacteria distribution and drug resistance in lung cancer patients with pulmonary infection, so as to provide basis for clinical rational drug use. Methods The clinical data of 392 lung cancer patients with pulmonary infection from September 2015 to August 2017 in the Affiliated Hospital of Hangzhou Normal University were analyzed retrospectively. 417 sputum samples were collected for pathogenic bacteria identification and drug sensitivity test, and the pathogenic bacteria distribution and drug resistance were analyzed statistically. Results A total of 133 strains of pathogenic bacteria were detected in sputum samples of 417 patients, the infection rate was 31. 89%,Among them, 87 strains of Gram-negative bacteria accounted for 65.41 %. Acinetobacter baumannii,Pseudomonas aeruginosa and Escherichia coli accounted for 24.06%, 16. 54% and 9. 02%.36 strains of Gram-positive bacteria accounted for 27. 07%,mainly Staphylococcus aureus and entero- coccus, accounting for 15. 79% and 11. 28%;10 strains of Fungi accounted for 7. 52%,The results showed that Gram-negative bacteria were sensitive to imipenem and amifloxacin. Gram-positive bacteria were highly sensitive to vancomycin and linezolid, and the sensitivity rate was 100. 00%,Conclusion Gram-negative bacteria is the main pathogenic bacteria in lung cancer patients with pulmonary infection. The drug resistance rate of pathogenic bacteria can be reduced by choosing reasonable antibiotics.
