超声造影实时动态观察特殊部位肝癌微波消融联合聚桂醇治疗的应用价值 被引量:8

摘要 目的探讨超声造影实时动态观察下特殊位置肝癌微波消融与聚桂醇联合治疗的应用价值。方法采用回顾性研究方法,选取2017年4月至2018年3月空军军医大学第一附属医院住院治疗的原发性肝癌患者120例,按照治疗方案不同将患者平均分为两组:观察组60例患者选择超声造影引导微波消融联合聚桂醇治疗,对照组60例患者选择超声造影引导微波消融治疗。对比两组患者的临床治疗效果以及并发症情况。结果观察组与对照组患者肿瘤灶数分别为92个、94个,穿刺数分别为174个、181个,消融位数分别为351个、357个,观察组肿瘤灶完全灭活率(超声造影:93.48%;增强CT:89.13%)明显高于对照组(超声造影:72.34%;增强CT:69.15%),差异具有统计学意义(P<0.05),超声造影与增强CT检查肿瘤灶完全灭活率差异无统计学意义(P>0.05)。观察组患者并发症发生率为61.67%,对照组为73.33%,两组相比差异无统计学意义(P>0.05)。结论超声造影实时动态观察下微波消融结合聚桂醇注射治疗特殊位置的原发性肝癌有良好的临床效果,肿瘤病灶的灭活率较高而并发症较少,超声造影实时观察肝癌病灶情况较为准确,对特殊位置肝癌治疗的临床价值较高。 Objective To explore the effect of microwave ablation combined with anhydrous ethanol on the real-time observation of liver cancer by contrast-enhanced ultrasonography.Methods A retrospective study was conducted in selection of 120 patients with primary liver cancer hospitalized in the First Affiliated Hospital of Air Force Military Medical University during April 2017 to March 2018.These patients were divided into two groups according to different treatment options:60 patients in observation group underwent ultrasound-guided microwave ablation combined with polyglycerol therapy,and 60 patients in control group underwent ultrasound-guided microwave ablation.The effect of clinical treatment and complications in patients of these two groups were compared.Results The amount of tumors in observation group and control group were 92 and 94 respectively.The times of punctures were 174 and 181 respectively.The times of ablation were 351 and 357,respectively.The complete inactivation rate of tumor in observation group was significantly higher than that of control group.There was no significant difference in learning(P<0.05)and complete inactivation rate between contrast-enhanced ultrasonography and enhanced CT scanning(P>0.05).The rate of complications was 61.67%in observation group and 73.33%in control group.There was no significant difference between these two groups(P>0.05).Conclusion Under the real-time dynamic observation of contrast-enhanced ultrasonography,microwave ablation combined with anhydrous ethanol injection have a good clinical effect in treatment of patients with primary liver cancer in special locality.The inactivation rate of tumor is high and the complications are less.The real-time observation on lesions of liver cancer by contrast-enhanced ultrasonography is more accurate,and it has a higher clinical significance for treatment of liver cancer in special locality.
