摘要
目的研究分析胸水结核菌核糖核酸(TB-RNA)、结核菌脱氧核糖核酸(TB-DNA)联合检测在结核性胸膜炎中的诊断价值。方法选取本院2018年3月至2018年12月期间收治的结核性胸膜炎40例患者作为观察组,选取同期收治的非结核性胸膜炎40例胸腔积液患者作为对照组。对两组胸水TB-RNA、TB-DNA进行检测,以细菌培养结果为金标准,分析胸水TB-RNA、TB-DNA联合检测在结核性胸膜炎中的诊断价值。结果TB-DNA检测结核性胸膜炎灵敏度62.50%,特异度87.50%,准确度75.00%,阳性预测值83.33%,阴性预测值70.00%,约登指数50.00%;TB-RNA检测结核性胸膜炎灵敏度72.50%,特异度90.00%,准确度81.25%,阳性预测值87.88%,阴性预测值76.60%,约登指数62.50%;联合检测对结核性胸膜炎灵敏度87.50%,特异度97.50%,准确度92.50%,阳性预测值97.22%,阴性预测值88.64%,约登指数85.00%;联合检测的灵敏度87.50%大于TB-DNA检测62.50%(P<0.05);联合检测的准确度92.25%大于TB-DNA检测75.00%、TB-RNA检测81.25%(P<0.05)。结论胸水TB-RNA、TB-DNA联合检测能有效提高结核性胸膜炎的诊断价值。
Objective To study and analyze the diagnostic value of combined detection of tuberculosis ribonucleic acid(TB-RNA)and tuberculosis bacterium deoxyribonucleic acid(TB-DNA)in pleural effusion in tuberculous pleurisy.Methods Forty patients with tuberculous pleurisy admitted in our hospital from March 2018 to December 2018 were selected as observation group,and 40 patients with non-tuberculous pleurisy admitted in the same period were selected as control group.TB-RNA and TB-DNA in pleural effusion of two groups were detected.The diagnostic value of combined detection of TB-RNA and TB-DNA in pleural effusion in tuberculous pleurisy was analyzed based on the results of bacterial culture as gold standard.Results The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of TB-DNA for tuberculous pleurisy were 62.50%,87.50%,75.00%,83.33%,70.00%and 50.00%,respectively.The sensitivity,specificity,accuracy,positive predictive value,negative predictive value and Yoden index of combined detection for tuberculous pleurisy were 87.50%,97.50%,92.50%,97.22%,88.64%,85.00%,87.50%and 62.50%respectively.The accuracy of combined detection was 92.25%,75.00%and 81.25%respectively(P<0.05).The sensitivity of combined detection was higher than that of TB-DNA detection(P<0.05).Conclusion Combined detection of TB-RNA and TB-DNA in pleural effusion can effectively improve the diagnostic value of tuberculous pleurisy.
