摘要
目的分析急性梗阻性化脓性胆管炎(AOSC)患者血清及胆汁中肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)的水平,并探讨血清TNF-α及IL-6水平诊断AOSC的价值。方法选择76例AOSC患者为观察组,76例非AOSC患者为对照组。均接受经内镜逆行胰胆管造影(ERCP)术治疗。比较两组患者术前血清及术中胆汁TNF-α、IL-6的水平。比较观察组患者术中及术后第1、3、7天胆汁中TNF-α、IL-6水平的变化。采用受试者工作特征(ROC)曲线分析血清TNF-α、IL-6水平对AOSC的诊断价值。结果观察组患者术前血清及术中胆汁TNF-α、IL-6的水平均高于对照组(均P<0.05)。观察组患者胆汁TNF-α、IL-6水平随时间呈下降趋势(均P<0.05)。术前血清TNF-α、IL-6水平诊断AOSC的ROC曲线下面积分别为0.732、0.709(均P<0.05)。结论AOSC发生时,血清及胆汁中的TNF-α、IL-6水平均升高;术前血清TNF-α、IL-6水平对AOSC均有一定的诊断价值。
Objective To analyze the levels of serum and bile tumor necrosis factorα(TNF-α)and interleukin 6(IL-6)in patients with acute obstructive suppurative cholangitis(AOSC),and to explore the diagnostic value of serum TNF-αand IL-6 for AOSC.Methods Seventy-six patients with AOSC were selected as observation group,and 76 patients without AOSC as control group.All patients received endoscopic retrograde cholangio-pancreatography(ERCP).The levels of TNF-αand IL-6 in serum and intraoperative bile were compared between the two groups.Changes in bile TNF-αand IL-6 during operation and on the 1st,3rd and 7th day after operation were compared in the observation group.The diagnostic value of serum TNF-αor IL-6 for AOSC was analyzed by receiver operating characteristic(ROC)curve.Results The observation group had higher levels of TNF-αand IL-6 in preoperative serum and intraoperative bile than the control group(all P<0.05).Bile level of TNF-αor IL-6 presented a tendency to decrease with time in the observation group(all P<0.05).The areas under ROC curve of preoperative serum TNF-αand IL-6 for diagnosing AOSC were 0.732 and 0.709,respectively(all P<0.05).Conclusion Patients experience elevated serum and bile TNF-αand IL-6 levels when suffering AOSC,both serum TNF-αand IL-6 levels before operation have a certain diagnostic value for AOSC.
