时间-空间相关成像-M型技术联合组织多普勒技术评价正常胎儿右心功能 被引量:5

Application of Spatio-Temporal Image Correlation-M Mode Technique and Tissue Doppler Imaging Technique in Evaluation of Right Ventricular Function of the Normal Fetuses

摘要 目的探讨时间-空间相关成像-M型(Spatio-Temporal Image Correlation-M,STIC-M)技术联合组织多普勒成像(Tissue Doppler Imaging,TDI)技术评价正常胎儿右心功能的应用。方法选取我院门诊及住院孕妇300例,孕龄20~40周,胎儿心内及心外结构未见明显异常;根据孕周分为4组:20^+1~24周(A组),24^+1~28周(B组),28^+1~32周(C组),32^+1~36周(D组),36^+1~40周(E组),每个孕周15例;应用STIC-M型技术测量各组胎儿三尖瓣环位移(Tricuspid Annular Displacement,TAD),应用TDI技术测量三尖瓣环舒张早期运动速度E'、舒张晚期运动速度A',并计算E'/A'。结果胎儿各组TAD、E'、E'/A'值与上一组比较,差异均有统计学意义(P<0.05);A^D组间A'值与上一组比较,差异均有统计学意义(P<0.05),D组与E组A'值比较,差异无统计学意义(P>0.05)。胎儿TAD、E'、A'、E'/A'值均与孕周呈正相关(P<0.05),即随着孕周的增加,TAD、E'、A'、E'/A'值增加,其中E'增加较快,A'增加较慢,E'/A'逐渐接近1。结论应用STIC-M型技术测量胎儿TAD联合TDI技术测量胎儿E'、A'、E'/A'值简便、易行,可及时了解胎儿右心收缩及舒张功能的改变,对早期发现胎儿心功能异常具有重要临床价值。 Objective To explore the application of spatio-temporal image correlation(STIC)-M mode technique and tissue Doppler imaging(TDI)technique in evaluation of right ventricular function of the normal fetuses.Methods A total of 300 pregnant women in our hospital were enrolled,whose gestational age ranged from 20 to 40 weeks.There was no obvious abnormalities in fetal intracardiac and extracardiac structures.According to gestational weeks,they were divided into four groups:20^+1 to 24 weeks(group A),24^+1 to 28 weeks(group B),28^+1 to 32 weeks(group C),32^+1 to 36 weeks(group D),36^+1 to 40 weeks(group E),15 cases per gestational week.Tricuspid Annular Displacement(TAD)was measured by STIC-M technique.The early and late diastolic myocardial velocity of tricuspid annulus(E',A')were measured by TDI technique,and the E'/A'was calculated.Results Compared to the previous group,the TAD,E',E'/A'in each group were significantly different(P<0.05).Compared to the previous group,the differences of the A'among the A to D groups were significantly different(P<0.05).The A'value had no statistical difference between the D group and the E group(P>0.05).The TAD,E',A'and E'/A'value were positively associated with gestational weeks(P<0.05).With the increase of the gestational weeks,the TAD,E',A'and E'/A'value were increased.The E'was increasing faster than the A'and E'/A'was near to 1.Conclusion It is a simple and feasible method to use STIC-M mode technique to measure TAD and to use TDI technique to measure the E',A',E'/A'of the fetuses,and can accurately evaluate fetal right ventricular systolic and diastolic function in time.Timely finding fetal cardiac dysfunction have important clinical significance.
