摘要
目的探讨髓源性抑制细胞(MDSCs)及调节性T细胞(Treg)在自身免疫性胰腺炎(AIP)患者体内的表达水平。方法选取海南省三亚市人民医院2014年5月至2018年5月收治的AIP患者106例,将其作为AIP组,选取同期于我院体检的健康体检者106例作为对照组。所有受检者均于入院当日采集血样检测外周血MDSCs、Treg水平。利用受试者工作特征曲线(ROC)分析血清MDSCs、Treg对AIP的预测价值,获得最佳截断值。收集AIP患者的临床特征,分析血清MDSCs、Treg表达水平与患者临床特征的关系。结果AIP组的MDSCs与Treg水平均高于对照组,差异有统计学意义(P<0.05)。MDSCs、Treg水平预测AIP的曲线下面积分别为0.671、0.708。MDSCs高表达组的胰腺外器官受累、IgG4异常占比分别为24.62%、63.08%,高于MDSCs低表达组的4.88%、31.71%,差异有统计学意义(P<0.05)。Treg高表达组的胰腺外器官受累、IgG4异常占比分别为25.00%、60.00%,高于Treg低表达组的6.52%、39.13%,差异有统计学意义(P<0.05)。结论与正常者相比,AIP患者的MDSCs、Treg水平明显上调,这可能与胰腺外器官受累、IgG4异常有关。
Objective To investigate the expression of Myelogenic suppressor cells(MDSCs)and regulatory T cells(Treg)in patients with autoimmune pancreatitis(AIP).Methods 106 patients with AIP admitted to our hospital from May 2014 to May 2018 were selected as AIP group.106 health examinees in our hospital during the same period were selected as control group.Blood samples were collected on the day of admission to detect the frequency and Treg level of MDSCs in peripheral blood.The ROC was used to analyze the predictive value of serum MDSCs frequency and Treg to AIP,and the best truncation value was obtained.To collect the clinical characteristics of AIP patients and analyze the relationship between serum MDSCs frequency,Treg expression and clinical characteristics of AIP patients.Results The frequency and Treg level of MDSCs in AIP group were higher than those in control group(P<0.05).The area under the curve of predicting AIP by MDSCs frequency and Treg level was 0.671 and 0.708,respectively.The proportion of extrapancreatic organ involvement and IgG4 abnormality in the high expression group of MDSCs were 24.62%and 63.08%,respectively,higher than that in the low expression group of MDSCs(4.88%and 31.71%)(P<0.05).The percentage of extrapancreatic organ involvement and IgG4 abnormality in Treg high expression group was 25.00%and 60.00%,respectively,higher than that in Treg low expression group(6.52%and 39.13%),(P<0.05).Conclusion Compared with normal subjects,the frequency of MDSCs and Treg level in AIP patients were significantly increased,which may be related to the involvement of extrapancreatic organs and abnormal IgG4.
