摘要
目的探讨凝血酶冻干粉治疗消化性溃疡出血的疗效及血小板参数变化。方法选取60例消化性溃疡出血患者,按随机数字表分为研究组(采用奥美拉唑联合凝血酶冻干粉治疗,30例)和对照组(采用奥美拉唑治疗,30例),分析对比两组患者的手术相关指标(止血时间、复发率、住院时间等)、止血率、临床治疗有效率、凝血指标、血小板参数变化等。结果研究组患者的止血时间(34.55±8.80)h、住院时间(7.00±1.60)d及复发率3.33%均低于对照组(60.30±8.00)h、(8.70±1.90)d、20.00%(均P<0.05),止血率96.67%、总有效率96.67%均高于对照组73.33%、76.67%(P<0.05);治疗后研究组血小板参数BPC(190.45±49.23)×10^9/L、PCT(0.26±0.07)%、MPV(6.39±0.56)fL和凝血指标PT(9.34±0.85)s、INR(0.87±0.15)、APTT(31.26±4.20)s、TT(14.14±1.86)s、Fg(5.02±1.76)g/L,均优于对照组(168.36±44.10)×10^9/L、(0.14±0.04)%、(8.50±0.70)fL、(12.42±0.96)s、(1.13±0.26)、(36.45±9.20)s、(16.23±2.35)s、(3.86±2.12)g/L,(P<0.05)。结论凝血酶冻干粉治疗消化性溃疡出血,可提高止血率,缩短止血时间及住院时间,并能显著改善血小板相关参数,效果显著,值得推广应用。
Objective To investigate the efficacy of thrombin freeze-dried powder in the treatment of peptic ulcer with hemorrhage and changes in platelet parameters.Methods Sixty patients with peptic ulcer with hemorrhage were randomly divided into study group(30 patients treated with omeprazole combined with thrombin lyophilized powder)and control group(30 patients treated with omeprazole).The surgical related indexes(hemostasis time,recurrence rate,hospitalization time,etc.),hemostasis rate,clinical treatment efficiency,coagulation index,and platelet parameter changes were compared between the two groups.Results Results The hemostasis time(34.55±8.80)h,hospitalization time(7.00±1.60)d and recurrence rate of 3.33%in the study group were lower than those in the control group(60.30±8.00)h,(8.70±1.90)d,20.00%(both P<0.05),hemostasis rate was 96.67%,total effective rate was 96.67%,which was higher than 73.33%and 76.67%of the control group(P<0.05).After treatment,the platelet parameters of the study group were BPC(190.45±49.23)×10^9/L,PCT(P<0.05).(0.26±0.07)%,MPV(6.39±0.56)fL and coagulation parameters PT(9.34±0.85)s,INR(0.87±0.15),APTT(31.26±4.20)s,TT(14.14±1.86)s,Fg(5.02±1.76)g/L,both superior to the control group(168.36±44.10)×10^9/L,(0.14±0.04)%,(8.50±0.70)fL,(12.42±0.96)s,(1.13±0.26),(36.45±9.20)s,(16.23±2.35)s,(3.86±2.12)g/L,(P<0.05).Conclusion Thrombin freeze-dried powder can improve hemostasis,shorten hemostasis time and hospitalization time,and can significantly improve platelet-related parameters.The effect is significant and worthy of popularization and application.
