摘要
目的比较宫颈鳞状上皮内病变的绝经患者行宫颈环形电切术(LEEP)前阴道使用雌二醇片/雌二醇地屈孕酮片复合包装与未使用患者的手术治疗效果的差异。方法回顾性分析医院2017年7月至2018年12月诊断为宫颈鳞状上皮内病变的绝经患者76例的临床资料,根据治疗方法分为两组。两组均行LEEP治疗,试验组26例术前使用雌二醇片/雌二醇地屈孕酮片复合包装塞阴道治疗2周,对照组50例术前未行任何药物处理,比较两组的治疗效果。结果试验组宫颈大小、LEEP深度、切除宫颈组织均大于对照组,差异均有统计学意义(P<0.05)。试验组锥切手术时间短于对照组,术中出血量少于对照组,疼痛程度评分低于对照组,差异均有统计学意义(P<0.05)。结论雌二醇片/雌二醇地屈孕酮片复合包装可以显著改善宫颈鳞状上皮内病变的绝经患者的阴道弹性,降低LEEP的难度,可切除更多的宫颈病变组织,取得更好的手术效果。
Objective To compare the therapeutic effects of compound packaging of estradiol tablets/estradiol-didrogesterone tablets in the anterior vagina of postmenopausal women undergoing cervical circumferential electrotomy(LEEP)with those of non-users.Methods The clinical data of 76 menopausal patients with cervical squamous intraepithelial lesions diagnosed in our hospital from July 2017 to December 2018 were retrospectively analyzed.They were divided into two groups according to the treatment methods.The two groups were treated with LEEP.26 cases in the experimental group were treated with estradiol tablets/estradiol-didrogesterone tablets combined with packing for vaginal plugging for 2 weeks before operation,while 50 cases in the control group were not treated with any drugs before operation.The therapeutic effects of the two groups were compared.Results The cervical size,LEEP depth and cervical tissue excision in the experimental group were larger than those in the control group,with statistical significance(P<0.05).The operation time of conization in the experimental group was shorter than that in the control group,the amount of bleeding during the operation was less than that in the control group,and the pain score was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Compound packaging of estradiol tablets/estradiol didrogesterone tablets can significantly improve the vaginal elasticity of postmenopausal women,reduce the difficulty of LEEP,remove more cervical lesions and achieve better surgical results.
