摘要
目的探讨川崎病中外周血核因子-κB(NF-κB)和基质金属蛋白酶-9(MMP-9),以及炎症因子的表达及意义。方法选取2015年1月至2016年1月在十堰市人民医院治疗的急性期川崎病患儿30例(KD组),同时选取正常小儿20例作为健康对照组,培养2组外周血单个核细胞(PBMCs),同时将KD组外周血PBMCs分为佛波酯组(PMA组)(20 nmol/L PMA),PMA+吡咯烷二硫代氨基甲酸(PDTC)组(在PMA基础上增加100 mol/L PDTC)和空白对照组(不作任何干预),采用免疫组化法检测NF-κB活化水平,ELISA法检测MMP-9、白细胞介素(IL)-6和IL-7水平。结果KD组NF-κB活化水平、MMP-9、IL-6和IL-7分别为(50.33±12.18)%、(904.18±83.20)ng/ml、(445.28±93.29)pg/ml和(36.81±9.43)pg/ml,明显高于健康对照组,差异有统计学意义(P<0.05);PMA组NF-κB活化水平、MMP-9、IL-6和IL-7明显高于空白对照组(P<0.05);PMA+PDTC组NF-κB活化水平、MMP-9、IL-6和IL-7较PMA组明显降低(P<0.05);KD组患儿NF-κB活化水平与IL-6和IL-7呈正相关(r=0.452和0.447,P<0.05);MMP-9与NF-κB活化水平、IL-6和IL-7无相关性(P>0.05)。结论KD患儿外周血PBMCs中MMP-9表达增强以及NF-κB活化在KD免疫性血管损伤中有重要作用,可为该病诊断、治疗、预后评估提供一定参考依据。
Objective To investigate the expression and clinical significance of peripheral blood nuclear factor-κB(NF-κB),matrix metalloproteinase-9(MMP-9)and inflammatory factor in Kawasaki disease(KD).Methods Thirty patients with Kawasaki disease in acute stage who were hospitalized from January 2015 to January 2016 in the hospital were enrolled for the study and were designated as the KD group,and 20 normal children were designated as the healthy control group.Peripheral blood mononuclear cells(PBMCs)were cultured for the 2 groups,and at the same time,PBMCs of the KD group were divided into the PMA group(20 nmol/L PMA),the PMA+PDTC group(plus 100 mol/L PDTC on the basis of PMA)and the blank control group(without any intervention).The level of NF-κB was detected by immunohistochemistry,and the levels of MMP-9,IL-6 and IL-7 were detected by ELISA.Results The levels of NF-κB,MMP-9,IL-6 and IL-7 in the KD group were respectively(50.33±12.18)%,(904.18±83.20)ng/ml,(445.28±93.29)pg/ml and(36.81±9.43)pg/ml,which were significantly higher than those of the healthy control group(P<0.05).The levels of NF-κB,MMP-9,IL-6 and IL-7 in the PMA group were significantly higher than those of the healthy control group(P<0.05).The levels of NF-κB,MMP-9,IL-6 and IL-7 in the PMA+PDTC group were significantly lower than those of the PMA group(P<0.05).In the patients of the KD group,the level of NF-κB was positively correlated with the levels of IL-6 and IL-7(r=0.452 and 0.447)(P<0.05).There was no correlation between the level of MMP-9 and the levels of NF-κB,MMP-9,IL-6 and IL-7(P>0.05).Conclusion The enhanced expression level of MMP-9 in PBMCs in the KD child patients and the activation of NF-κB played an important role in immune vascular injury,and they could provide evidence for the diagnosis,treatment and prognosis of the disease.
