摘要
目的探讨在经皮肾镜钬激光碎石术患者中应用丙泊酚复合腰硬联合麻醉的效果。方法选取2017年12月~2018年12月在我院实施经皮肾镜钬激光碎石术的60例患者作为研究对象,随机分为对照组和观察组,每组各30例。对照组患者应用全身麻醉,观察组患者应用丙泊酚复合腰硬联合麻醉。比较两组的麻醉效果,并比较两组患者术中的血流动力学变化。结果观察组的麻醉起效时间、完全阻滞时间、自主恢复时间和定向力恢复均短于对照组,差异有统计学意义(P<0.05);观察组麻醉后5 min(T1)、麻醉后10 min(T2)的平均动脉压(MAP)、心率(HR)均低于对照组,差异有统计学意义(P<0.05);两组麻醉前(T0)、麻醉后30 min(T3)、手术结束时(T4)的MAP、HR比较,差异无统计学意义(P>0.05)。结论在经皮肾镜钬激光碎石术患者中应用丙泊酚复合腰硬联合麻醉具有更好的麻醉效果,且术后恢复快,对血流动力学的影响更小。
Objective To investigate the effect of Propofol plus combined spinal-epidural anesthesia in patients undergoing percutaneous nephrolithotomy with holmium laser lithotripsy.Methods A total of 60 patients who underwent percutaneous nephrolithotomy with holmium laser lithotripsy in our hospital from December 2017 to December 2018 were selected as the subjects.They were randomly divided into control group and observation group,with 30 cases in each group.The control group was given general anesthesia,while the observation group was given propofol plus combined combined spinal-epidural anesthesia.The anesthetic effect of the two groups was compared,and the hemodynamic changes during operation were compared between the two groups.Results The onset time of anesthesia,complete block time,autonomous recovery time and directional recovery of the observation group were shorter than those of the control group,and the differences were statistically significant(P<0.05).The mean arterial pressures(MAP)and heart rates(HR)at 5 min after anesthesia(T1)and 10 min after anesthesia(T2)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in MAP and HR at before anesthesia(T0),30 min after anesthesia(T3)and the end of surgery(T4)between the two groups(P<0.05).Conclusion Propofol plus combined spinal-epidural anesthesia has better anesthetic effect in patients undergoing percutaneous nephrolithotomy with holmium laser lithotripsy,which has faster recovery and less influence on hemodynamics.
