摘要
目的观察针灸治疗老年膝骨关节炎的有效性及安全性。方法将120例膝骨关节炎患者按年龄分为中年组(40~59岁)、老年组(60~79岁),组内再分为塞来昔布组与针灸组,观察治疗前后患者的疼痛、膝关节功能活动、安全性。结果(1)中年针灸组、中年塞来昔布组、老年针灸组、老年塞来昔布组患者治疗后2w、4w、12w对膝骨关节炎的疼痛视觉模拟评分(VAS)及膝关节功能活动评分(西安大略麦马斯特大学骨性关节炎量表,WOMAC)与治疗前比较均降低(P<0.05);(2)与中年塞来昔布组相比,中年针灸组患者在治疗后4w,VAS和WOMAC评分降低,且差异有统计学意义(P<0.05);与老年塞来昔布组相比,老年针灸组患者在治疗后4w、12w WOMAC评分降低,差异有统计学意义(P<0.05),且老年塞来昔布组有2例患者出现胃部不适症状。结论针灸可缓解中老年患者膝骨关节炎疼痛、改善功能活动。3个月随访发现针灸对关节功能恢复疗效优于塞来昔布。
Objective To explore the clinical curative of acupuncture on knee osteoarthritis(KOA)in the elderly and the safety.Methods 120 cases with KOAwere divided,according to the age,into 2 groups:middle-aged group(40~59 years old)and elderly group(60~79)years old,and the 2 groups were further divided into 4 sub-groups:middle-aged celecoxib group,middle-aged acupuncture group,elderly celecoxib group and elderly acupuncture group;Visual Analogue Scale(VAS)and Western Ontarioand McMaster Universities Arthritis Index(WOMAC)were applied in assessing the degree of pain and the knee joint function before and after treatment;the VAS scores,WOMAC scores and the safety of acupuncture were observed.Results The VAS scores and WOMAC scores of all the cases in the 4 groups decreased significantly 2,4,and 12 weeks after treatment(P<0.05);the VAS scores and WOMACscores of cases in middle-aged acupuncture group were significantly lower than those in middle-aged celecoxib group 4 weeks after treatment(P<0.05),and the same in the 2 sub-groups of elderly group(P<0.05);stomach discomfort was found in 2 cases in elderly celecoxib group.Conclusions Acupuncture is of a curative effect on pain relief and joint function improvement,the 3months follow-up showed that the effect of acupuncture was superior to that of celecoxib group.
