摘要
目的观察老年抑郁症患者针刺太冲穴前后静息态功能磁共振(fMRI)变化,探讨针刺太冲穴对功能异常脑区的调节作用。方法选择男性老年抑郁症患者15例,安静平卧于扫描床上,选取右侧太冲穴垂直直刺,平补平泻手法,行针2 min后拔针。于针刺操作前8 min和操作后8 min,使用3.0 T磁共振功能成像技术进行磁共振扫描,采集静息态fMRI数据,利用DPABI软件测算患者治疗前后的局部一致性(ReHo)指标,分析其变化情况。结果与针刺前比较,针刺后ReHo值明显降低的脑区为左侧颞中下回、右侧枕下回、右侧颞中回、左侧眶部额下回、左侧枕中回、左侧角回、双侧额中上回、左侧楔叶、顶上小叶;针刺后ReHo值明显升高的脑区为左侧颞下回、左侧岛叶、双侧海马及海马旁回。结论老年抑郁症患者针刺太冲穴后可直接影响认知、情绪和视觉相关脑区,进而调节患者异常皮层的功能活动。
Objective To explore the changes of resting-state functional magnetic resonance imaging(r-fMRI)before and after acupuncture at right Taichong point in elderly patients with depression,and to investigate the regulation of acupuncture at Taichong point on the abnormal brain area.Methods The resting state data of 15 elderly depression patients at 8 min before treatment and 8 min after acupuncture at right Taichong point were collected by using 3.0 T magnetic resonance functional imaging technique,and the regional homogeneity(ReHo)indexes were calculated and analyzed statistically.Results Compared with those before acupuncture,the brain areas with decreased ReHo values after acupuncture were the left inferior temporal gyrus,right suboccipital gyrus,right middle temporal gyrus,left orbital inferior frontalis gyrus,left middle occipital gyrus,left angular gyrus,bilateral middle and superior frontal gyrus,left cuneiform lobe,and superior parietal lobe;the brain areas with increased ReHo values after acupuncture were the left inferior temporal gyrus,left insular lobe,bilateral hippocampus and parahippocampal gyrus.Conclusion Acupuncture at Taichong point can directly affect cognitive,emotional and visual related brain areas,and then regulate the abnormal cortex function in elderly patients with depression.
