摘要
目的:观察针灸联合活血利水方治疗腰椎间盘突出症的临床疗效。方法:选择2017年4月至2018年5月本院治疗的腰椎间盘突出症患者112例,按照数字随机表法分为治疗组和对照组,每组56例。治疗组给予针灸联合活血利水方口服治疗,对照组给予单纯针灸治疗。比较两组患者治疗前后疼痛视觉模拟评分(visual analogue scale,VAS)、日本骨科协会腰椎功能评分(JOA)、血浆黏度、全血高切黏度、全血低切黏度和血清肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、前列腺素E_2(prostaglandin E_2,PGE_2)和白细胞介素-6(interleukin-6,IL-6)水平。结果:治疗组治疗后VAS评分、血浆黏度、全血高切黏度、全血低切黏度和血清TNF-α、PEG_2、IL-6水平均低于对照组,差异有统计学意义(P<0.01);治疗组治疗后JOA评分高于对照组,差异有统计学意义(P<0.01);治疗组有效率94.64%,对照组有效率78.57%,治疗组优于对照组,差异有统计学意义(Z=6.247,P=0.007<0.01)。结论:针灸联合活血利水方治疗腰椎间盘突出症疗效显著,能有效缓解疼痛,改善患者腰椎功能,降低血液黏稠度,促进炎症因子吸收。
Objective:To observe the clinical curative effect of acupuncture and moxibustion in combination with Decoction for Promoting Blood Circulation and Diuresis on lumbar disc herniation.Methods:A total of 112 patients with lumbar disc herniation treated in our hospital from April 2017 to May 2018 were selected and divided into the treatment group and the control group according to the method of digital random table,with 56 cases in each group.The treatment group was given acupuncture and moxibustion combined with Decoction for Promoting Blood Circulation and Diuresis,while the control group was given acupuncture and moxibustion alone.Then the levels of visual analogue scale(VAS),Japanese Orthopaedic Association(JOA)lumbar spine function score,plasma viscosity,whole blood high shear viscosity,whole blood low shear viscosity and serum tumor necrosis factor-α(TNF-α),prostaglandin E2(PGE2)and interleukin-6(IL-6)were compared between the two groups before and after treatment.Results:After the treatment,the VAS score,plasma viscosity,whole blood high shear viscosity,whole blood low shear viscosity and serum TNF-α,PEG2,IL-6 levels in the treatment group were lower than those in the control group,and all the differences were statistically significant(P<0.01).After the treatment,the JOA score of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.01).The effective rate of the treatment group was 94.64%,and that of the control group was 78.57%.The treatment group was better than the control group,and the difference was statistically significant(Z=6.247,P=0.0070.01).Conclusion:Acupuncture and moxibustion in combination with Decoction for Promoting Blood Circulation and Diuresis has a remarkable efficacy on lumbar disc herniation,can effectively relieve pain,improve lumbar function,reduce blood viscosity and promote the absorption of inflammatory factors.
