早期肠内营养在重症高血压脑出血并发肺部感染治疗中的应用价值分析 被引量:6

The Value of Early Enteral Nutrition in the Treatment of Severe Hypertensive Cerebral Hemorrhage Complicated with Pulmonary Infection

摘要 目的观察早期肠内营养(EEN)对于重症高血压脑出血并发肺部感染患者的应用价值。方法用“随机数字表分配法”于2016年3月—2018年10月选取该院68例重症高血压脑出血并发肺部感染患者作为研究对象,将他们分为对照组(34例)和观察组(34例)两组,根据患者的实际情况为其选择合适的治疗方式(手术治疗或保守治疗),在此基础上,对照组患者实施常规营养支持治疗,观察组患者实施EEN治疗,对比两组患者治疗前后的血液营养指标及肺部炎症指标。结果治疗前两组患者的各项血液营养指标及肺部炎症指标均差异无统计学意义(P>0.05),治疗后对照组患者的前白蛋白、白蛋白及血红蛋白水平(213.06±32.15)mg/L、(38.59±6.87)g/L、(106.32±12.26)g/L均明显低于观察组(253.42±35.21)mg/L、(43.41±7.29)g/L、(115.64±14.62)g/L,C-反应蛋白及白细胞水平[(40.86±8.05)mg/L、(13.26±3.67)×10^9/L]均比观察组患者高[(19.41±4.52)mg/L、(9.01±3.02)×10^9/L](P<0.05)。结论对重症高血压脑出血并发肺部感染患者实施早期肠内营养支持可改善其营养状况,降低肺部感染的严重程度,值得大力推广实施。 Objective To observe the value of early enteral nutrition(EEN)in patients with severe hypertensive cerebral hemorrhage complicated with pulmonary infection.Methods Sixty-eight patients with severe hypertensive cerebral hemorrhage complicated with pulmonary infection in the hospital from March 2016 to October 2018 were selected as“study randomized digital table allocation method”.They were divided into two groups:control group(34 cases)and observation group(34 cases).The actual condition of the patient was chosen according to the appropriate treatment(surgical treatment or conservative treatment).On this basis,the patients in the control group were treated with conventional nutritional support,and the patients in the observation group were treated with EEN.The blood nutrition indexes before and after treatment were compared between the two groups.And lung inflammation indicators.Results There were no statistically significant differences in blood nutrition index and lung inflammation index between the two groups before treatment(P>0.05).Pre-albumin,albumin and hemoglobin levels in the control group after treatment(213.06±32.15)mg/L,(38.59±6.87)g/L,(106.32±12.26)g/L were significantly lower than the observation group(253.42±35.21)mg/L,(43.41±7.29)g/L,(115.64±14.62)g/L,C-reactive protein And white blood cell levels[(40.86±8.05)mg/L,(13.26±3.67)×10^9/L]were higher than those in the observation group[(19.41±4.52)mg/L,(9.01±3.02)×10^9/L](P<0.05).Conclusion Early enteral nutrition support for patients with severe hypertensive cerebral hemorrhage complicated with pulmonary infection can improve their nutritional status and reduce the severity of lung infection,which is worthy of promotion.
