摘要
目的:探讨嗜酸性粒细胞衍生神经毒素(EDN)对儿童呼吸道过敏性疾病的诊断价值及其与血清总IgE(tIgE)水平的相关性。方法:诊断为过敏性哮喘或(和)过敏性鼻炎的65例患儿作为病例组,健康儿童22例为对照组,根据病例组患儿的血清tIgE水平将其分为A组(42例,血清tIgE水平>200 kU/L)和B组(23例,血清tIgE水平为100~200 kU/L)。对各组患者的血tIgE、EDN、嗜酸性粒细胞阳离子蛋白(ECP)、高迁移率族蛋白B1(HMGB1)水平进行检测和比较。结果:病例A组、病例B组、对照组三组间比较,血清ECP水平的差异无统计学意义(P>0.05),血清EDN和HMGB1水平的差异有统计学意义(P<0.05)。A、B组间患儿血清EDN和HMGB1水平的差异无统计学意义(P>0.05),但均高于对照组,差异均有统计学意义(P<0.05)。受试者工作特征曲线(ROC曲线)分析结果显示,血清EDN水平诊断儿童呼吸道过敏性疾病的ROC曲线下面积(AUC)具有统计学意义(P<0.05),AUC值为0.894,而血清ECP、HMGB1水平诊断儿童呼吸道过敏性疾病的AUC无统计学意义(P>0.05)。直线相关分析和多元线性回归分析结果显示,患儿的血清EDN水平与tIgE水平具有相关性(P<0.05)。结论:相对于ECP和HMGB1,血清EDN对儿童呼吸道尘螨过敏性疾病具有更高的实验诊断价值,其表达水平与IgE水平具有显著相关性,有望作为儿童呼吸道过敏性疾病的辅助诊断和病情评价的新型生物学指标。
Objective:To analyze the value of eosinophil-derived neurotoxin(EDN) in diagnosis of respiratory allergic diseases in children and its correlation with total IgE(tIgE) levels.Methods: Sixty-five children with allergic asthma or/and allergic rhinitis were selected as the case group,and 22 healthy children were selected as the control group.The children in the case group were divided into two groups according to the serum tIgE levels,which were Group A(42 cases,serum tIgE level >200 kU/L) and Group B(23 cases,serum tIgE level was 100-200 kU/L).The serum levels of tIgE,EDN,eosinophil cationic protein(ECP) and high mobility group protein B1(HMGB1) in each subject were measured and compared.Results: There was no significant difference for the serum ECP level among the Group A,Group B,and the control group(P>0.05).The differences of the serum EDN and HMGB1 levels were statistically significant(P<0.05).There were no significant differences for the serum EDN and HMGB1 levels between Group A and Group B(P>0.05),but the levels were both higher than those in the control group(P<0.05).The receiver operating characteristic curve(ROC curve) analysis showed that the area under ROC curve(AUC) of the serum EDN level was 0.894,which was statistically significant in the diagnosis of respiratory allergic diseases in the children(P<0.05),while those of ECP and HMBG1 levels were not statistically significant in diagnosis of these diseases(P>0.05).Linear correlation analysis and multiple linear regression analysis showed that serum EDN level was correlated with tIgE level(P<0.05).Conclusion: Compared with ECP and HMGB1,the serum EDN is more valuable in diagnosis of allergic diseases in children′s respiratory tracts.The expression level of EDN is significantly correlated with the level of serum tIgE,which is expected to be used as a new biological indicator for auxiliary diagnosis and evaluation of allergic diseases in the respiratory tract in children.
