摘要
目的建立衰弱老年人急性期快速恢复病房(acute care for the elderly unit,ACE unit),评价其对老年病房医疗指标的影响。方法通过建立ACE单元,对急性疾病期衰弱老年人实施多学科团队照护,比较老年病房在ACE单元建立前后2年医疗指标的变化。结果ACE单元建立后,老年病房的平均住院日由建立前的(21.14±2.24)d缩短至(18.33±3.93)d(P=0.004),人均住院费用由(2.48±0.31)万元减少至(2.15±0.26)万元(P<0.001),药占比由(40.50±3.31)%降低至(29.00±3.58)%(P<0.001);实际占用床日数、病床周转次数、床位占用率、每月出院人次差异无统计学意义(P>0.05)。结论ACE单元的建立能改善老年病房的医疗指标,提高老年病房的医疗质量,是急性疾病期衰弱老年人住院期间照护的成功模式。
Objective To establish an acute care for the elderly unit(ACE unit),and to evaluate its impact on the medical indicators of geriatric wards.Methods Through the establishment of ACE unit,multi-disciplinary team care was carried out for the elderly with frailty and acute disease,and the changes of medical indicators in the elderly ward before and after the establishment of ACE unit were compared.Results After the establishment of the ACE unit,the average hospitalization time of the geriatric ward was shortened from 21.14±2.24 days before the establishment to 18.33±3.93 days(P=0.004),and the per capita hospitalization cost was reduced from 24,800±3,100 RMB to 21,500±2,600 RMB(P<0.001),and drug-expenditure proportion was decreased from(40.50±3.31)%to(29.00±3.58)%(P<0.001).The actual number of beds occupied,number of bed turnover,bed occupancy rate and monthly discharges were not significantly different before and after the establishment of the ACE unit(P>0.05).Conclusions The establishment of ACE unit can improve the medical indicators of geriatric wards and improve the medical quality of geriatric wards.It is a successful mode of care for elderly patients with debilitating acute disease.
