摘要
目的探讨鼻中隔-鼻骨矫正术治疗外伤性鼻骨骨折合并畸形的临床疗效及对鼻腔通气水平的影响。方法选取自2015年1月至2019年6月收治的60例外伤性鼻骨骨折合并畸形患者为研究对象。术后随访1年,采用视觉模拟评分(VAS)评估术前、术后的鼻外形、鼻通气功能、嗅觉障碍评分。检测患者术前、术后鼻腔最小横截面积、鼻腔容积、鼻总呼气量。采用Pearson分析鼻腔通气VAS评分与鼻腔最小横截面积、鼻腔容积、鼻总呼气量的相关性。评估患者术前、术后生活质量,包括生理机能、生理职能、情感职能、社会功能、精神健康、总体健康。观察患者术后临床疗效及并发症发生情况。结果患者均行1年门诊随访,无失访患者,随访完成率100.0%(60/60)。本组患者显效57例(95.0%),有效3例(5.0%),总有效率100.0%(60/60)。患者手术均一次性成功,无鼻隔脓肿、血肿、再次塌陷等并发症发生。术后,患者鼻外形评分、鼻通气功能、嗅觉障碍评分均较术前降低,差异有统计学意义(P<0.05)。术后,患者鼻腔最小横截面积、鼻腔容积、鼻总呼气量均较术前增加,差异有统计学意义(P<0.05)。患者术后鼻腔通气VAS评分与鼻腔最小横截面积、鼻腔容积、鼻总呼气量均呈正相关(r=0.368,r=0.419,r=0.334,P<0.05)。术后,患者生理机能、生理职能、情感职能、社会功能、精神健康、总体健康评分均较术前明显升高,差异有统计学意义(P<0.05)。结论鼻中隔-鼻骨矫正术治疗外伤性鼻骨骨折合并畸形患者临床疗效显著,可恢复患者鼻腔通气功能。
Objective To investigate the effect of septum-nasal bone correction on traumatic fracture of nasal bone combined with deformity and the effect of nasal ventilation.Methods A retrospective study was performed on 60 cases of patients with traumatic nasal fracture combined with deformity who were admitted from January 2015 to June 2019.Visual analogue score(VAS)was used to evaluate preoperative and postoperative nasal appearance,nasal ventilation function and olfactory dysfunction.The minimum cross sectional area,nasal volume and total nasal exhalation were measured before and after surgery.Pearson was used to analyze the correlation between VAS score and minimum cross-sectional area,nasal volume and total nasal expiratory volume.Assess the patients′preoperative and postoperative quality of life,including physiological function,physiological function,emotional function,social function,mental health and overall health.The postoperative clinical efficacy and complications were observed.Results All patients were followed up for 1 year in outpatient department,and no patients were lost to follow-up.The follow-up completion rate was 100.0%(60/60).The total effective rate was 100.0%(60/60).The operation was successful without any complications such as septal abscess,hematoma and secondary collapse.Postoperative nasal shape score,nasal ventilation function and olfactory dysfunction score of the patients were lower than that of the patients before surgery,with statistically significant differences(P<0.05).After surgery,the minimum cross-sectional area,nasal volume and total nasal expiratory volume of the patients were all increased compared with that before surgery(P<0.05).VAS score of postoperative nasal ventilation was positively correlated with the minimum cross-sectional area,nasal volume and total nasal expiratory volume(r=0.368,r=0.419,r=0.334,P<0.05).After surgery,the patients′physiological function,physiological function,emotional function,social function,mental health and overall health were all significantly improved compared with that before surgery(P<0.05).Conclusion Nasal septum-nasal bone correction for traumatic nasal bone fracture combined with deformity of the clinical effect is significant,can restore the nasal ventilation function of patients.
