摘要
目的探讨封闭负压引流(VSD)+超声清创手术(UDM)治疗糖尿病足溃疡的应用及临床预后。方法纳入该院80例2017年1月-2018年12月糖尿病足溃疡患者。随机数字表分组,常规换药组采取常规换药治疗,VSD+UDM组则采取常规换药加上VSD+UDM治疗。比较常规换药组、VSD+UDM组疗效;创面平均愈合的时间、换药的平均次数、平均住院时间以及住院费用;治疗前后患者经皮氧分压、创面面积、足背动脉血流速度、胫后神经传导速度。结果 VSD+UDM组治疗效果高于常规换药组(χ~2=7.923,P<0.05)。其中,常规换药组治疗显效17例,治疗有效12例,治疗无效11例,治疗总有效率是72.50%。VSD+UDM组治疗显效34例,治疗有效4例,治疗无效2例,治疗总有效率是95.00%。VSD+UDM组创面平均愈合的时间、换药的平均次数、平均住院时间以及住院费用(24.41±1.67)d、(10.21±1.24)次、(23.21±1.56)d、(2.21±1.21)万元低于常规换药组(t=7.945、6.834、5.821、5.821,P<0.05)。结论常规换药加上VSD+UDM治疗糖尿病足溃疡的效果确切,可有效提高足背动脉血流的平均速度、胫后神经传导的平均速度,缩小溃疡,加速创面愈合,有效提高经皮氧分压,减少换药次数和缩短住院时间,减少医疗费用。
Objective To investigate the application and clinical prognosis of closed vacuum drainage(VSD) + ultrasonic debridement surgery(UDM) in the treatment of diabetic foot ulcer. Methods 80 patients with diabetic foot ulcers from January 2017 to December2018 in the hospital. The random number table was grouped, the conventional dressing group was treated with conventional dressing,and the VSD+UDM group was treated with conventional dressing plus VSD+UDM. The efficacy of the conventional dressing group,VSD+UDM group;the average healing time of the wound, the average number of dressing changes, the average hospital stay, and the hospitalization cost;the percutaneous oxygen partial pressure, the wound area, the dorsal artery blood flow velocity before and after treatment, The rate of nerve conduction after the sac. Results The therapeutic effect of the VSD+UDM group was higher than that of the conventional dressing group (χ~2=7.923, P<0.05). Among them, there were 17 cases in which the conventional dressing group was effective, 12 cases were effective, 11 cases were ineffective, and the total effective rate was 72.50%. There were 34 cases in the VSD+UDM group, 4 cases in the treatment, and 2 cases in the treatment. The total effective rate was 95.00%. The average healing time of the wounds in the VSD+UDM group, the average number of dressing changes, the average hospitalization time, and the hospitalization expenses were(24.41±1.67)d,(10.21±1.24)times,(23.21±1.56)d,(2.21±1.21)million yuan lower than the conventional dressing group(t=7.945,6.834,5.821,5.821,P<0.05). Conclusion Conventional dressing plus VSD+UDM is effective in treating diabetic foot ulcers, which can effectively improve the average velocity of the dorsal artery blood flow, the average speed of posterior tibial nerve con-duction, reduce ulcers, accelerate wound healing, and effectively improve transepidermal oxygen. Partial pressure, reduce the number of dressings and shorten the length of hospital stay, reduce medical expenses.
