摘要
目的观察经皮撬拨复位克氏针内固定治疗儿童JudetⅣ型桡骨颈骨折的临床疗效。方法对2015年11月至2018年1月郑州市骨科医院小儿骨科收治的19例JudetⅣ型桡骨颈骨折患儿采用经皮撬拨复位克氏针内固定术治疗,术后应用长臂石膏托外固定,观察手术时间、术中出血量、并发症发生情况、骨折愈合时间及愈合效果。结果19例患儿的平均手术时间为20.4 min、术中出血量为2.9 mL;术后所有患儿均未出现感染、骨化性肌炎、尺桡骨近端融合、肘关节不稳定等并发症,骨折均获骨性愈合,平均愈合时间为4.5周,肘关节屈伸及旋转功能均恢复正常,平均恢复时间为11d;随访12个月时,19例患儿经Metaizeau功能评价标准评定的临床疗效均为良好。结论经皮撬拨复位克氏针内固定治疗儿童JudetⅣ型桡骨颈骨折具有操作简便、创伤小、肘关节功能恢复好等优势,值得临床推广应用。
Objective To observe the curative effect of percutaneous prying reduction and Kirschner wire internal fixation in the treatment of Judet type IV radial neck fracture in children.Methods Nineteen children patients with Judet type IV radial neck fracture,admitted into the Pediatric Orthopedics Department of Zhengzhou Orthopedics Hospital between November 2015 and January 2018 were treated with percutaneous prying reduction and kirschner wire internal fixation,followed by the postoperative application of long arm plaster slab for external fixation.The operative time,intraoperative blood loss,complication occurrence,fracture healing time and healing effect were observed.Results In the 19 children patients,the mean operative time was 20.4 min and the mean intraoperative blood loss was 2.9 ml.After the operation,no patients developed complications such as infection,myositis ossificans,proximal ulnar and radial fusion,elbow instability and etc.All the fractures of patients realized bone healing,and the mean healing time was 4.5 weeks.The flexion and rotation functions of elbow joint returned to normal and the mean recovery time was 11 d.During the twelve-month follow-up visit,the curative effect of the 19 patients was assessed as good according to Me taizeau’s classification.Conclusion The application of percutaneous prying reduction and Kirsohner wire internal fixation in the treatment of Judet type IV radial neck fracture in children presented the advantages of simple and easy operation,minimal invasiveness and good recovery of elbow functions,deserving to be promoted in clinical practice.
