摘要
目的观察管氏舌针与常规针刺治疗缺血性中风后失语患者的临床疗效。方法将60例中风后失语患者随机分为观察组和对照组,各30例。2组均采用常规药物治疗。观察组根据管氏舌针穴位图选取聚泉、中矩、金津玉液、心穴、肝穴、脾穴、肾穴点刺,其中肝穴、脾穴、肾穴均取双侧,每穴针刺5 s,采用平补平泻手法,不留针,每天治疗1次。对照组则采用常规针刺,选取廉泉、风池、通里、阴陵泉、太冲,每次留针30 min,采用平补平泻手法,每天治疗1次,2组均治疗20 d,2个疗程。观察2组患者治疗前后功能性语言沟通能力检查法(CFCP)评分、失语商、改良的汉语成语成套测验ABC评分,运用改良的失语症严重程度分级标准(BDAE)评价疗效。结果通过治疗后,2组复述、自发谈话语言功能测定均较治疗前明显升高(P<0.05),且观察组明显高于对照组(P<0.05);2组对失语严重程度的改善均具有明显性差异(P<0.05),且观察组在改善失语的严重程度上均优于对照组(P<0.05)。结论管氏舌针改善缺血性中风后失语患者临床症状优于常规针刺;可改善中风后失语患者的日常生活对语言的沟通能力。
Objective:To observe the clinical effects of Guan's tongue and conventional acupuncture in the treatment of aphasia patients after ischemic stroke.Methods:60 aphasia patients after stroke were randomly divided into an observation group and a control group,30 cases each group.Both groups were treated with conventional drugs.The observation group was given acupuncture at the points of Juquan,Zhongmo,Jinjin Yuye,heart,liver,spleen and kidney according to Guan's tongue acupoints map.The liver,spleen and kidney points were taken from each side,acupuncturing at each point for 5 s.The technique of smoothing and replenishing was used,no needle kept,once a day.The control group was treated with conventional acupuncture at the points of Lianquan,Fengchi,Tongli,Yinlingquan and Taichong and the needles were kept for 30 min each time.The technique of smoothing and replenishing was used and the treatment was once a day.Both groups were treated for 20 days with 2 courses of treatment.The Chinese functional communication profile(CFCP)score,the aphasia quotient and the improved Chinese idiom test ABC score were observed before and after treatment and the modified Boston Diagnostic Aphasia Examination(BDAE)were used to evaluate the curative effect.Results:After treatment,the rehearsal and spontaneous speech language function of the two groups were significantly higher than those before treatment(P<0.05),and the observation group was significantly higher than the control group(P<0.05).The improvement of the severity of aphasia in the two groups showed significant differences(P<0.05),and the observation group was superior to the control group in improving the severity of aphasia(P<0.05).Conclusion:Guan's tongue acupuncture is better than conventional acupuncture in the improvement of clinical symptoms of aphasia patients after ischemic stroke and can improve the communication ability of daily life to language in patients with aphasia after stroke.
