摘要
目的观察上颌前牙行超声骨刀微创拔牙后即刻种植的美学效果。方法选取2015年5月-2018年5月期间收治的上颌前牙需拔除且同时行种牙手术的患者60例,随机分为观察组和对照组各30例,观察组给予超声骨刀拔牙后即刻种植,对照组给予常规微创拔牙后即刻种植。比较两组患者拔牙手术时间,术后出血、疼痛感、面部肿胀情况、种植体稳定性、骨边缘水平、白色美学指数评分(WES)、红色美学指数评分(PES)及患者满意情况。结果观察组患者拔牙手术时间显著长于对照组,术后出血情况发生率低于对照组(均P<0.05);观察组患者术后1 d疼痛程度及术后3 d面部肿胀程度均低于对照组(χ2=23.951,P=0.000;χ2=9.178,P=0.002);观察组患者ISQ、周围探诊深度、远中侧骨吸收量、近中侧骨吸收量与对照组相比无统计学差异(P>0.05);观察组患者的PES评分显著高于对照组(P<0.05);观察组患者治疗后的总满意度96.67%显著优于对照组的80.00%(χ2=4.043,P=0.042)。结论超声骨刀微创拔牙能够显著降低术中及术后并发症,同期即刻种植后美学效果及患者的满意度也较高,值得推广应用。
Objective To observe aesthetic effect of immediate implantation on maxillary anterior teeth after minimally invasive extraction with piezosurgery.Methods 60 patients who needed to undergo maxillary anterior teeth extraction and tooth implantation,and were admitted to the hospital from May 2015 to May 2018 were enrolled.They were randomly divided into observation group and control group,30 cases in each group.The observation group was given immediate implantation after piezosurgery extraction,while control group was given immediate implantation after routine minimally invasive extraction.The duration of extraction,postoperative bleeding,pain sense,facial swelling,implant stability,bone margin level,white esthetics score(WES),pink esthetics score(PES),and patients satisfaction were compared between the two groups.Results The duration of extraction in observation group was significantly longer than that in control group,while incidence of postoperative bleeding was lower than that in control group(P<0.05).The pain degree at 1d after surgery and facial swelling degree at 3d after surgery in observation group were lower than those in control group(χ2=23.951,P=0.000.χ2=9.178,P=0.002).There was no significant difference in ISQ,depth of peripheral probing,amount of bone resorption at the distal and middle side or proximal and middle side(P>0.05).PES of observation group was significantly higher than that of control group(P<0.05).After treatment,total satisfaction of observation group was significantly better than that of control group(96.67%vs 80.00%)(χ2=4.043,P=0.042).Conclusion Piezosurgery minimally invasive extraction can significantly reduce intraoperative and postoperative complications.After synchronous immediate implantation,aesthetic effect and patients satisfaction are also high.
