FISH技术在监测膀胱尿路上皮癌术后复发的临床价值 被引量:1

摘要 目的利用FISH技术检测膀胱尿路上皮癌术后尿脱落细胞的染色体畸变特点,探讨该技术在膀胱尿路上皮癌复发中的临床价值。方法采用荧光标记的3、7、17和9号染色体探针检测60例膀胱尿路上皮癌患者术后尿液,分析FISH结果与肿瘤复发的关联性。结果随访6~24个月,平均15个月,随访期间明确复发者32例,复发率53.3%,其中FISH阳性者27例(84.4%),FISH阴性5例(15.6%);膀胱镜检查未见肿瘤复发28例,其中FISH阳性10例(35.7%),FISH阴性18例(64.3%)。FISH阳性患者出现肿瘤复发率高达73%(27/37),FISH阴性患者肿瘤复发率为21.3%(5/23),差异有统计学意义(P<0.05)。结论FISH结果阳性提示肿瘤复发的风险增加,为临床评估肿瘤复发提供参考依据。 Objective To explore the application of fluorescence in situ hybridization(FISH)technique in detection of bladder cancer patients with postoperative urine exfoliated cells,discuss FISH technology in application value in predicting recurrence of bladder cancer.Methods The fluorescentlabeled probes of chromosomes 3,7 and 17 were used to detect the postoperative urine of 60 patients with bladder urothelial carcinoma,and to analyze the correlation between FISH results and tumor recurrence.Results The patients were followed up for 6-24 months after FISH detection,with an average follow up of 15 months,a total of 32 cases(53.3%)with tumor recurrence were examined during the follow up period with histopathological diagnosis,including FISH positive 27 cases(84.4%),and negative in 5 cases(15.6%).Patients with no tumor recurrence were found in 28 cases,in which the FISH positive 10 cases(35.7%),negative 18 cases(64.3%),FISH positive patients 73.0%(27/37)were found of tumor recurrence,while the proportion of FISH negative patients were found of tumor recurrence in only 21.3%(5/23),with the statistically significant difference(P<0.05).Conclusion FISH is in the diagnosis of bladder cancer and have great significance recurrence monitoring of bladder cancer,has important clinical value,can provide the reference for the recurrence of bladder cancer.
