摘要
目的分析抗苗勒管激素(AMH)在多囊卵巢综合征(PCOS)中的诊断价值。方法选取2016年1月至2018年6月就诊于本院生殖中心的105例PCOS患者作为试验组,150例体检健康育龄期女性作为对照组。分别测定两组血清AMH、体质量指数(BMI)、雌二醇(E2)、卵泡刺激素(FSH)、黄体生成素(LH)、孕酮(P)及睾酮(T),比较两组内分泌代谢指标的差异,同时借助受试者工作曲线分析AMH、LH、T对PCOS的临床评估价值。根据AMH水平高低分为高AMH组(AMH≥10.23 ng/ml,n=62)和低AMH组(AMH<10.23 ng/ml,n=43),分别测定两组空腹血糖(FPG)、空腹胰岛素(INS)、体质量指数(BMI),计算胰岛素抵抗指数(HOMA-IR),分析PCOS患者中不同水平AMH对糖代谢指标的影响。结果研究组AMH、LH、T均显著高于对照组,差异均有统计学意义(均P<0.05)。高AMH组的血清AMH、HOMA-IR、INS、BMI均高于低AMH组,差异均具有统计学意义(均P<0.05)。血清AMH诊断PCOS的AUC值为0.823,显著高于LH、T的AUC值;当其cut-off值为6.95 ng/ml时,诊断PCOS的敏感度为85.23%,特异度为72.54%。结论AMH在PCOS患者中表达高水平,且与糖代谢指标呈正相关性,可为今后PCOS诊断和研究提供新思路。
Objective To analyze the diagnostic value of anti-Mullerian hormone for polycystic ovary syndrome.Methods A total of 105 PCOS patients treated in our hospital's reproductive center from January,2016 to June,2018 were selected as an experimental group,and 150 healthy women of childbearing age as a control group.The serum AMH,body mass index(BMI),estradiol(E2),follicle stimulating hormone(FSH),luteinizing hormone(LH),progesterone(P),and testosterone(T)were measured in the two groups.The differences in endocrine and metabolic indices between the two groups were compared.The clinical value of AMH,LH,and T in PCOS was analyzed with the help of the working curve of the subjects.According to the level of AMH,the patients were divided into a high AMH group(AMH≥10.23 ng/ml,n=62)and a low AMH group(AMH<10.23 ng/ml,n=43).The fasting blood glucose(FPG),insulin(INS),and body mass index(BMI)were measured respectively;and the insulin resistance index(HOMA-IR)were calculated to analyze the effects of different levels of AMH on glucose metabolism in the patients.Results The AMH,LH,and T of the study group were significantly higher than those of the control group(all P<0.05).The serum AMH,HOMA-IR,INS,and BMI of the high AMH group were higher than those of the low AMH group(all P<0.05).The AUC value of serum AMH for the diagnosis of PCOS was 0.823,which was significantly higher than the AUC values of LH and T.Given that the cut-off value was 6.95 ng/ml,the sensitivity of diagnosis of PCOS was 85.23%,and the specificity 72.54%.Conclusion AMH is highly expressed in patients with PCOS and positively correlated with glucose metabolism index,which may provide new ideas for the diagnosis and research of PCOS in the future.
