细小脲原体感染量与胎膜早破及新生儿肺炎的相关性分析 被引量:5

摘要 目的探讨细小脲原体(UP)与胎膜早破及新生儿肺炎的相关性。方法随机选取2016年1月至2017年10月在深圳市罗湖区妇幼保健院住院分娩的产妇及其所产新生儿205对,对产妇产前宫颈分泌物及新生儿口咽部分泌物采用实时荧光定量PCR法检测UP含量,结合病历综合分析UP感染载量与胎膜早破及新生儿肺炎发生率的相关性。结果①产妇UP阳性与UP阴性比较,UP感染未增加胎膜早破发生率,UP阳性胎膜早破发生率似更低(χ~2=4.164,P<0.05),且感染载量与胎膜早破无明显直接相关性(r=0.124,P>0.05),反之,有胎膜早破与无胎膜早破的UP垂直传播率比较差异无统计学意义(校正χ~2=0.110,P>0.05);②产妇宫颈分泌物的UP载量与新生儿肺炎发生率无明显直接相关性(r=0.732,P>0.05),仅与垂直传播率呈正相关性(r=0.866,P<0.05);③新生儿口咽部分泌物UP阳性与UP阴性相比,新生儿肺炎发生率差异有统计学意义(校正χ~2=8.889,P<0.01),且新生儿口咽部UP感染载量与新生儿发生率呈正相关(r=0.908,P<0.05)。结论产妇UP感染与胎膜早破无明显直接相关,胎膜早破亦未增加UP垂直传播风险;产妇宫颈分泌物的UP载量越高,垂直传播率越高;新生儿UP感染载量与新生儿肺炎发生率呈正相关,对低载量感染患儿予以密切关注,对感染载量在10~5及以上者尽早干预,可能会更有效地降低不明原因的新生儿肺炎发生率。 Objective To further explore the correlation of ureaplasma parvum(UP) with premature rupture of fetal membranes and neonatal pneumonia.Methods 205 pairs of maternity patients and neonates who were hospitalized at Luohu District Maternal and Child Healthcare Hospital during the period from January,2016 to October 2017 were randomly selected.Detection of UP in maternal cervical secretions and neonatal oropharyngeal secretions was performed by real-time fluorescent quantitative PCR method.Combined with medical records,the correlation of the amount of UP infection with the incidence of premature rupture of membranes and neonatal pneumonia was analyzed.Results 1) Comparison between positive UP group and the negative UP group,UP infection didn’t increase the rate of the premature rupture of fetal membranes,and the rate of premature rupture of positive UP group seemed to be even lower(χ2=4.164,P<0.05).And there was no significant direct correlation between infection load and premature rupture of membranes(r=0.124,P>0.05).Conversely,the difference in UP vertical transmission rate between the group with early rupture of membranes and the group with no premature-rupture of membranes was not statistically significant(after correction,χ2=0.110,P>0.05).2) There was no significant direct correlation between UP quantity in maternal cervical secretions and the incidence of neonatal pneumonia(r=0.732,P>0.05).But it was showed a positive correlation with the vertical transmission rate(r=0.866,P<0.05).3) Comparison between the positive UP oropharyngeal secretions and negative UP secretions,the difference in the incidence of neonatal pneumonia between them showed statistical significance(after correction,χ2=8.889,P<0.01),and UP infection quantity of neonatal oropharynx was positively correlated with the incidence rate of neonatal pneumonia(r=0.908,P<0.05).Conclusion There was no obvious correlation between ureaplasma parvum infection and the incidence of premature rupture of membranes,and neither did premature rupture of membranes increase the risk of ureaplasma parvum vertical transmission.The higher the UP quantity of maternal cervical secretions was,the higher the vertical transmission rate was.The UP quantity of neonatal oropharynx was positively correlated with the incidence of neonatal pneumonia.Close attention should be paid to babies with low quantity of UP.Early intervention to those babies whose number of UP was 105 and above,and this may reduce more effectively the incidence of neonatal pneumonia of unknown origins.
