结核分枝杆菌及利福平耐药实时荧光定量核酸扩增技术在结核病诊断中的价值 被引量:6

摘要 目的评估结核分枝杆菌及利福平耐药实时荧光定量核酸扩增(Gene Xpert MTB/RIF)技术在结核病诊断中的应用价值。方法选取2017年1月-2019年1月北京和睦家医院收治的首次就诊的疑似结核病患者92例。收集患者标本,分别进行涂片抗酸染色镜检、液体分枝杆菌培养和Gene Xpert MTB/RIF检测。通过临床确诊结核病例来评价Gene Xpert MTB/RIF技术的敏感度和特异度。结果92例疑似结核病患者中经临床诊断10例确诊为结核病,涂片抗酸染色镜检、液体分枝杆菌培养和Gene Xpert MTB/RIF检测阳性率分别为5.4%(5/92)、12.0%(11/92)、8.7%(8/92)。涂片抗酸染色镜检敏感度和特异度为30.0%和97.6%,液体分枝杆菌培养敏感度和特异度为70.0%和95.1%,Gene Xpert MTB/RIF敏感度和特异度为80.0%和100%。三种方法相对于临床诊断结果,关联性有统计学意义(P<0.05),差异无统计学意义(P>0.05)。结论Gene Xpert MTB/RIF技术有较高的敏感度和特异度,对结核病的早期诊断和及时治疗有重要的应用价值。 Objective To evaluate the diagnostic value of Gene Xpert MTB/RIF test for tuberculosis.Methods A total of 92 patients who were suspected of having tuberculosis(TB)at their first visit to Beijing United Family Hospital from January 2017 to January 2019 were selected.Specimens of the patients were collected for acid-fast smear microscopic examination,liquid mycobacterium culture and Gene Xpert MTB/RIF detection.The sensitivity and specificity of Gene Xpert MTB/RIF technique by clinical diagnosis of tuberculosis was evaluated.Results Among 92 cases with suspected tuberculosis,10 cases were diagnosed as TB.The positive rate of acid-fast smear microscopy,liquid mycobacterial culture and Gene Xpert MTB/RIF was 5.4%(5/92),12.0%(11/92)and 8.7%(8/92),respectively,and the sensitivity and specificity of acid-fast smear microscopy was 30%and 97.6%,it was 70%and 95.1%for liquid mycobacteria,80.0%and 100.0%for MTB/RIF.The reliability analysis including agreement and consistency,both confirmed the three methods were all consistent with clinical diagnosis.Conclusion Gene Xpert MTB/RIF technology shows high sensitivity and specificity,which has important value for early diagnosis and timely treatment of tuberculosis.
