摘要
目的 探讨沙库巴曲缬沙坦对于射血分数降低的心力衰竭(HEr EF)患者,治疗6个月后超声上左心房内径(LA)、左心室舒张末期内径(LVDL)、射血分数及N端脑利钠肽前体NT-proBNP水平上的治疗效果评价。方法 选取2017年11月-2018年12月在本院住院接受诊疗的HEr EF患者59例作为研究对象,均给予指南推荐的个体化的心衰药物治疗方案,病情稳定后完善超声心动图、NT-proBNP测定,同时加用沙库巴曲缬沙坦起始50 mg口服,每日2次,每2周剂量倍增1次,出院后定期随访,根据个体血压情况加至个体最大耐受量,每日2次口服。出院后规律随访,分别在1个月、3个月及6个月复查超声心动图及NT-proBNP。结果 出院后1个月、3个月及6个月查超声心动图结果示左房、左室舒张末期内径、射血分数及NT-pro BNP数值均较住院期间病情稳定时好转,但数据仅6个月的射血分数及NT-proBNP数值对比住院期间病情稳定时的具有统计学差异(P<0.05)。结论 长期运用沙库巴曲缬沙坦能一定程度上改善HEr EF患者的心室重塑、显著改善射血分数及NT-proBNP水平,从而改善HEr EF患者的预后。
Objective This study investigates the effects of sacubitril valsartan on heart function in heart failure patients with reduced ejection fraction(HFr EF)after six month of treatment,through the detection of left atrial diameter(LA),left ventricular end-diastolic dimension(LVDD),ejection fraction and N-terminal-pro-B-type natriuretic peptide(NT-pro BNP).Methods 59 patients with HFr EF hospitalized in our hospital from November,2017 to December,2018 were selected as study subjects.All of them were given the individualized drug treatment recommended by the guidelines for patients with heart failure.In a stable condition,they were consummated the echocardiography,and NT-pro BNP was detected.During the same period the patients were treated with sacubitril valsartan orally twice daily at the dose of 50 mg,and them the dose was doubled every 2 weeks until the target dose was reached to individual maximum tolerance according to the individual blood pressure.The patients were regularly followed up after discharged,echocardiography and NT-pro BNP were recorded on 1 st month,3 rd month and 6 th month respectively after discharged.Results Echocardiography results at 1 stmonth,3 rdmonth and 6 thmonth after discharge showed that the left atrium diameter,left ventricular enddiastolic dimension and ejection fraction and NT-pro BNP were better than those during they were in hospital.But only the ejection fraction and NT-pro BNP at 6 thmonth were statistically different from those during they were in hospital(P<0.05).Conclusions Long term administration of sacubitril valsartan may improve the heart structure restructure,effectively improve ejection fraction and NT-pro BNP level,so that improve the prognosis of patients with HEr EF.
