摘要
目的探讨人造石英石作业人员的石英粉尘暴露对肺功能的影响。方法选取2017年9月至2018年3月,湖北省中西医结合医院职业病门诊及呼吸内科门诊就诊的46名男性石英石作业人员为暴露组,采用同期体检中心体检的41名健康男性为对照组。比较两组研究对象肺通气及弥散功能检查结果。结果暴露组工人肺活量(vital capacity,VC);用力肺活量(forced vital capacity,FVC);第1秒用力呼气量(forced expiratory volume at the first second of FVC,FEV1);最大呼气流速(peak expiratory flow,PEF)以及25%肺活量最大呼气流速(maximum expiratory flow at 25percent,MEF25%);50%肺活量最大呼气流速(maximum expiratory flow at 50percent,MEF50%);75%肺活量最大呼气流速(maximum expiratory flow at 75percent,MEF75%)的实测值占预计值百分率均低于对照组,差异有统计学意义(U=619.5、637.5、572.0、547.5、549.0、536.5、582.5,P=0.006、0.009、0.002、0.001、0.001、0.001、0.002)。两组间FEV1/FVC差异无统计学意义(U=732.0,P=0.073)。暴露组工人肺一氧化碳弥散量(diffusion capacity for carbon monoxide of the lung,DLCO)小于对照组,差异无统计学意义(U=825.5,P=0.318)。结论人造石英石粉尘的呼吸道暴露可导致工人肺通气功能明显下降,并以限制性通气功能障碍为主。
Objective To describe the effect of synthetic quartz stone dust exposure on pulmonary function of workers.Methods The pulmonary function tests were performed in 46workers occupationally exposed to synthetic quartz stone dust and had doctor visits in occupational disease clinics and pulmonary disease clinics in Hubei Provincial Hospital of Integrated Chinese and Western Medicine as exposure group,as well as in 41agematched healthy individuals as controls who did annual physical check-up in physical examination center during September 2017and March 2018.The test results of pulmonary ventilation function and pulmonary diffusion function were analyzed and compared between the two groups.Results The vital capacity(VC),forced vital capacity(FVC),forced expiratory volume at the first second of FVC(FEV1),peak expiratory flow(PEF),maximum expiratory flow at 25percent(MEF25%),maximum expiratory flow at 50percent(MEF50%)and maximum expiratory flow at 75percent(MEF75%)of subjects in the exposure group were significantly lower than that of controls(U=619.5,637.5,572.0,547.5,549.0,536.5,582.5,P=0.006,0.009,0.002,0.001,0.001,0.001,0.002).There was no statistical difference in FEV1/FVC between the two groups(U=732.0,P=0.073).Although the diffusion capacity for carbon monoxide of the lung was lower in exposure group,the difference was not statistically significant(U=825.5,P=0.318).Conclusions Exposure to synthetic quartz dust can damage the pulmonary ventilation function of workers,predominately the limited ventilation dysfunction.
