摘要
目的探究和分析肺泡灌洗液二代测序对肺部感染性疾病的诊断价值.方法选取2018年4月-2019年6月期间我院收治的肺部感染性疾病的患者的临床资料总计43例,收集所有患者的肺泡灌洗液进行二代测序(mNGS),对检测的结果进行分析,评价对于肺部感染性疾病的诊断的价值.结果43例肺部感染性疾病的患者中,检测出病原体38例(88.4%),单一病原体感染:其中检出病毒5例(11.6%),支原体5例(11.6%),流感嗜血杆菌3例(7.0%),铜绿假单胞菌3例(7.0%),副流感嗜血杆菌:2例(4.7%),肺炎链球菌2例(4.7%),屎肠球菌2例(4.7%),大肠杆菌2例(4.7%),曲霉菌2例(4.7%),结核分枝杆菌2例(4.7%),鲍曼不动杆菌2例(4.7%),其他6例(13.9%),未检出5例(11.6%).混合感染:副流感嗜血杆菌+卡氏肺虫孢子1例(2.3%),副流感嗜血杆菌+病毒1例(2.3%).经过检测的结果进行抗感染治疗后,所有患者均顺利出院.结论肺泡灌洗液二代测序对病原体的检测较为敏感、诊断较为准确,对于针对性地使用抗生素有很好的参考价值,值得在临床推广.
Objective To explore and analyze the diagnostic value of alveolar lavage fluid second-generation sequencing in pulmonary infectious diseases. Methods Clinical data of 42 patients with pulmonary infectious diseases admitted to our hospital from April 2018 to June 2019 were selected. All patients’ alveolar lavage fluid was collected for second-generation sequencing(mNGS), and the detection results were analyzed to evaluate the diagnostic value of pulmonary infectious diseases. Results among the 42 patients with pulmonary infectious diseases, 40 patients(95.2%)were infected with a single pathogen: staphylococcus aureus was detected in 2 patients(4.8%), but no one(2.4%). Mycoplasma pneumoniae was found in 18 cases(42.9%), virus in 15 cases(35.7%) and other 4 cases(9.5%). Mixed infection: mycoplasma pneumoniae + virus in 2 cases(4.8%), mycoplasma pneumoniae + haemophilus influenzae in 1 case(2.4%). After anti-infection treatment, all patients were discharged successfully. Conclusion the second generation sequencing of alveolar lavage fluid is sensitive to pathogen detection and accurate in diagnosis, which is of good reference value for targeted use of antibiotics and worthy of clinical promotion.
