摘要
目的分析左炔诺孕酮宫内缓释系统联合固定式宫内节育器(吉妮柔适环)对子宫腺肌病患者痛经、月经异常的临床疗效。方法选取2014年1月~2018年12月东莞市寮步医院妇科收治的120例子宫腺肌病患者为研究对象,随机分为两组,各60例。对照组患者单独采用放置左炔诺孕酮宫内缓释系统(LNGIUS,商品名曼月乐)进行治疗,观察组患者采用左炔诺孕酮宫内缓释系统联合固定式宫内节育器(吉妮柔适环)治疗,分析结果。结果放置前,两组受试者的痛经评分、月经量以及子宫内膜厚度、CA125,差异无统计学意义(P> 0.05)。放置后第3、6以及12个月观察组上述指标改善更为显著,不良反应发生率低于对照组,差异有统计学意义(P <0.05)。结论左炔诺孕酮宫内缓释系统联合固定式宫内节育器(吉妮柔适环)可全面减少子宫腺肌病患者的月经量,有效缓解痛经症状,积极调节血清中CA125水平,降低不良反应发生率,提高患者的依从性,治疗费用少,其在治疗子宫腺肌疾病方面有着一定参考价值,适合在基层医院推广应用。
Objective To analyze the clinical effect of levonorgestrel intrauterine slow-release system combined with fixed intrauterine device(Gini Ring) on dysmenorrhea and abnormal menstruation in patients with adenomyosis. Methods 120 cases of adenomyosis were randomly divided into two groups,60 cases in each group,from January 2014 to December 2018 in the Department of Gynecology,Liaobu Hospital.The patients in the control group were treated with LNGIUS alone,while the patients in the observation group were treated with LNG-IUS combined with fixed intrauterine device(Gini Soft Ring). Results Before intervention,there was no significant difference in dysmenorrhea score,menstrual volume,endometrial thickness and CA125 between the two groups(P > 0.05).The incidence of adverse reactions and compliance between the two groups were compared,and the incidence of adverse reactions in the observation group was significantly lower than that in the control group,with statistically significant differences(P < 0.05). Conclusion Levonorgestrel intrauterine slow-release system combined with fixed intrauterine device(Gini Roushihuan) can reduce the menstrual volume of adenomyosis patients,effectively relieve dysmenorrhea symptoms,actively regulate the level of CA125 in serum,and reduce the incidence of adverse reactions.It has certain reference value in the treatment of diseases of uterine adenomyosis and is suitable for popularization and application in grass-roots hospitals.
