摘要
目的探讨分娩球配合自由体位助产对巨大儿产妇盆底功能的影响。方法根据随机分组的原则将2017年12月至2018年12月我院经B超结合临床提示胎儿体质量≥4000 g的180例产妇分为两组,每组90例。对照组接受常规的卧位待产和分娩,实验组实施分娩球和自由体位相结合方式待产和分娩。比较两组产程、产钳应用、产道裂伤情况、Ⅰ类和Ⅱ类肌纤维相关情况、盆腔脏器脱垂(POP)、尿失禁(SUI)等情况。结果实验组产钳使用比例、会阴Ⅲ度裂伤、会阴侧切、宫颈裂伤、POP、SUI发生率均比对照组低,差异有统计学意义(P <0.05);实验组第一产程、第二产程、总产程、Ⅰ类和Ⅱ类肌纤维疲劳度均较对照组低,Ⅰ类和Ⅱ类肌纤维阴道压力较对照组高(P <0.05)。结论分娩球联合自由体位,可有效改善巨大儿产妇妊娠结局,缩短产程,减轻对盆底肌肉损伤,维持盆底功能,减少盆底功能障碍性疾病。
Objective To investigate the effect of childbirth ball combined with free position on midwifery function in large children. Methods According to the principle of uniform grouping, 180 pregnant women who were combined with B-ultrasound and fetal weight ≥4000 g in December 2017-December 2018 were divided into two groups, namely, control group and experimental group, 90 cases in each group. The control group received routine supine position and delivery, and the experimental group performed a combination of delivery ball and free body position for labor and delivery. The two groups of labor, forceps application, birth canal laceration, type I and II muscle fiber related conditions, pelvic organ prolapse(POP), urinary incontinence(SUI) were compared. Results The incidence of forceps, perineal III degree laceration, perineal lateral incision, cervical laceration, POP and SUI in the experimental group were lower than those in the control group(P<0.05). The experimental group had the first stage of labor and the second stage. The fatigue of labor, total labor, type I and type II muscle fibers were lower than those of the control group. The vaginal pressure of type I and type II muscle fibers was higher than that of the control group(P<0.05). Conclusion The birthing ball combined with free body position can effectively improve the maternal pregnancy outcome, shorten the labor process, reduce the damage to the pelvic floor muscles, maintain the pelvic floor function, reduce the pelvic floor dysfunction disease.
