摘要
目的探讨老年股骨颈骨折患者应用髋关节置换术治疗的临床疗效。方法纳入2018年6月至2019年6月石河子大学医学院第一附属医院连续收治的老年股骨颈骨折患者110例,随机分为观察组(56例)和对照组(54例),其中观察组采用髋关节置换术治疗,对照组采用骨折内固定术治疗,比较分析两组的治疗效果及并发症发生情况。结果观察组患者手术时间和住院时间明显长于对照组,卧床时间明显短于对照组,差异均有显著性(P<0.001)。治疗前两组髋关节功能评分差异无显著性(P>0.05),术后2周观察组患者髋关节功能评分明显优于对照组(P<0.01)。观察组术后并发症发生率为3.57%,明显低于对照组的14.81%,差异有显著性(P=0.003)。结论髋关节置换术治疗老年股骨颈骨折可以缩短卧床时间,减少术后并发症,有助于髋关节功能恢复。
Objective To investigate the clinical effect of hip replacement in the treatment of femoral neck fracture in the elderly.Method A total of 110 elderly patients with femoral neck fracture admitted to our hospital from June 2018 to June 2019 were selected in this study.Fifty-six of them were treated with hip replacement(experimental group)and 54 with fracture internal fixation(control group).To analyze the clinical admission characteristics and surgical treatment effect of the experimental group and the control group.Result The length of operation and hospitalization in the experimental group was longer than that in the control group(both P<0.001),but the length of bed rest was significantly lower than that in the control group.There was no significant difference in hip function score between the experimental group and the control group before operation(P>0.05).Two weeks after operation,the hip function score of the control group was significantly higher than that of the experimental group(P<0.01).The total incidence of complications was 3.57%in the experimental group and 3.57%in the control group.The incidence was 14.81%.The incidence of complications in the experimental group was significantly lower than that in the control group(P=0.003).Conclusion Hip replacement for femoral neck fracture in the elderly can shorten the time of bed rest,reduce the complications after operation,and contribute to the recovery of hip function.
