河源地区无偿献血人群乙型肝炎病毒感染标志物检测结果分析 被引量:2

Analysis of test results of hepatitis B virus infection markers in voluntary blood donors in Heyuan

摘要 目的探讨河源地区实施核酸检测后无偿献血人群乙型肝炎病毒感染标志物检出情况,为血液安全策略提供参考。方法对2016年1月~2018年12月我站无偿献血者样本的HBsAg及HBV DNA检测结果进行统计分析,对不合格样本按献血者献血年份、年龄和是否重复献血进行分组比较。对128例HBsAg-/HBV DNA+样本进行QPCR病毒载量测定。结果河源地区60351例无偿献血样本中检出HBsAg 667例,检出率为1.11%;57594例ELISA及ALT检测结果阴性的样本中检出HBsAg-/HBV DNA+128例,检出率为0.22%。不同年份HBsAg、HBsAg-/HBV DNA+检出率差异均无统计学意义(χ^2=3.846、8.402;P=0.145、0.150)。不同年龄、不同献血次数间的HBsAg检出率差异均有统计学意义(χ^2=51.513、492.800;P均=0.000)。不同年龄、不同献血次数间HBsAg-/HBV DNA+检出率差异均有统计学意义(χ^2=34.217、15.617;P均=0.000)。128例HBsAg-/HBV DNA+样本,其中95例病毒载量范围为0.210~1383.929IU/mL,中位数26.786 IU/mL,其余33例均不可定量。结论河源地区无偿献血者乙肝病毒感染标志物检出率较高,ELISA联合NAT检测有效降低了输血传播HBV风险。从固定献血人群特别是低年龄段人群采集血液,可有效降低输血传播HBV风险。 Objective To explore the test results of hepatitis B virus(HBV)infection markers in voluntary blood donors in Heyuan after completing nucleic acid detection,and to provide reference for blood safety strategy.Methods The hepatitis B surface antigen(HBsAg),HBV and deoxyribonucleic acid(DNA)test results of voluntary blood donors in our station from January 2016 to December 2018 were statistically analyzed,and the unqualified samples were grouped and compared according to the Year,age and repeated blood donation.The viral load of 128 HBsAg-/HBV DNA+samples was determined using quantitative polymerase chain reaction(QPCR).Results In 60351 voluntary blood samples from Heyuan District,667 cases of HBsAg were detected,and the detection rate was 1.11%;57594 samples with negative results of ELISA and ALT,128 cases of HBsAg-/HBV DNA+were detected,and the detection rate was 0.22%.There was no significant difference in the detection rate of HBsAg and HBsAg-/HBV DNA+in different yea rs(χ^2=3.846,P=0.145;χ^2=8.402,P=0.150).The detection rate of HBsAg in different ages and times of blood donation was statistically significant(χ^2=51.513,P=0.000;χ^2=492.800,P=0.000).The detection rate of HBsAg-/HBV DNA+in different ages and times of blood donation was statistically significant(χ^2=34.217,P=0.000;χ^2=15.617,P=0.000).Among 128 HBsAg-/HBV DNA+samples,95 cases had viral load ranging from 0.210 to 1383.929 IU/mL,with a median of 26.786 IU/mL,and the rest 33 cases were not quantifiable.Conclusion The detection rate of HBV infection markers in voluntary blood donors is higher in Heyuan,and enzyme-linked immunosorbent assay(ELISA)combined with nucleicacid testing(NAT)detection can effectively reduce the risk of HBV transmission in blood transfusion.Collecting blood from the fixed blood donors,especially the low age group,can effectively reduce the risk of HBV transmission.
