布地奈德和吸入用复方异丙托溴铵溶液联合雾化吸入治疗慢性阻塞性肺病急性加重期的疗效观察 被引量:14

摘要 目的探讨布地奈德、吸入用复方异丙托溴铵溶液联合雾化吸入治疗慢性阻塞型肺病急性加重期(AECOPD)的疗效。方法选取本院2016年9月至2018年9月收治的80例慢性阻塞型肺疾病患者,随机分为两组,各40例。对照组患者在常规化痰、平喘、镇咳治疗的基础上给予布地奈德雾化治疗,观察组在对照组的基础上给予吸入用复方异丙托溴铵溶液雾化治疗,比较两组患者临床疗效、肺功能指标、血气分析指标以及不良反应发生情况。结果观察组患者治疗有效率为92.50%高于对照组的72.50%(P<0.05);观察组患者治疗后1 s用力呼气末容积占正常预计值百分比(FEV1%)、1 s用力呼气末容积与用力肺活量比值(FEV1/FVC)、最大呼气流量占正常预计值百分比(PEF%)水平高于对照组(P<0.05);观察组患者治疗后二氧化碳分压(PaCO2)、动脉血氧饱和度(SaO2)低于对照组,动脉血氧分压(PaO2)水平高于对照组(P<0.05);两组患者不良反应发生率比较差异无统计学意义。结论布地奈德、吸入用复方异丙托溴铵溶液联合雾化吸入治疗AECOPD,可提高患者肺功能,改善血气分析治疗,疗效显著,用药安全性较高。 Objective To investigate the therapeutic effect of budesonide and Corbitt combined with atomization inhalation on acute exacerbation of chronic obstructive pulmonary disease(AECOPD). Methods 80 patients with chronic obstructive pulmonary disease from September 2016 to September 2018 in this hospital were randomly divided into two groups, with 40 cases in each group. The control group was given budesonide atomization treatment on the basis of routine phlegm, asthma, and cough therapy. The observation group was treated with kobit atomization on the basis of the control group, and the clinical efficacy, pulmonary function index, blood gas analysis index and adverse reactions of the two groups were compared. Results The effective rate of 92.50% in observation group was higher than that of control group 72.50%(P<0.05). The observation group with 1 second of forced exhalation end volume as a percentage of normal estimated value(FEV1%), 1 second of forced exhalation end volume and force lung activity ratio(FEV1/FVC), and maximum exhalation flow as a percentage of normal estimated value(PEF%) were higher than control group(P<0.05). After treatment, carbon dioxide partial pressure(PaCO2) and arterial blood oxygen saturation(SaO2) were lower in the observation group than in the control group, and arterial blood oxygen partial pressure(PaO2) was higher than that in the control group(P<0.05). There was no significant difference in the incidence of ADR between the two groups. Conclusion The combination of budesonide and Corbitt atomization inhalation therapy AECOPD can improve lung function, blood gas analysis and treatment, and the efficacy and safety of drug use are remarkable.
