摘要
目的:研究瘘道造影联合X射线摄影在新生儿先天性肛门闭锁诊断中的应用价值。方法:选取医院收治的102例先天性肛门闭锁新生儿,患儿均接受B超、X射线检查,针对伴瘘道的患儿行瘘道造影。以手术结果为金标准,分析不同检查方式对高位、中位及低位肛门闭锁的检出情况,并计算相应的灵敏度、特异度和准确率,分析X射线联合瘘道造影诊断与手术诊断的一致性。结果:经手术结果提示,在102例患儿中低位肛门闭锁39例(占38.24%),中位肛门闭锁34例(占33.33%),中位肛门闭锁29例(占28.43%)。X射线联合瘘道造影诊断低位肛门闭锁与手术诊断一致性为0.766,高于B超诊断的0.583;X射线联合瘘道造影诊断中位肛门闭锁与手术诊断一致性为0.727,高于B超诊断的0.651;X射线联合瘘道造影诊断高位肛门闭锁与手术诊断一致性为0.670,高于B超诊断的0.472。结论:采用瘘道造影联合X射线诊断新生儿先天性肛门闭锁的效果优于常规B超,且与手术诊断的一致性更好。
Objective:To explore the applied value that fistulography combined with X-ray in the diagnosis of congenital anal atresia in neonates.Methods:102 neonates with congenital anal atresia admitted to hospital were selected.All the neonates underwent B-mode ultrasoundand X-ray examination.Fistulography was performed for children with fistula.The results of operation was used as gold standard to analyze the detection results of different examination method on atresia at high,middle and low position,and to calculatethe corresponding sensitivity,specificity and accuracy,and to analyze the consistency of fistulography with X-ray and surgical diagnosis.Results:The results of surgery indicated that 39 cases were anal atresiaat low position(38.24%),34 cases were anal atresia at middle position(33.33%)and 29 cases were anal atresia at high position(28.43%).In the diagnosis of anal atresia at low position,the consistency of the diagnosis of X-ray with fistulography and surgical diagnosis was 0.766,which was higher than 0.583 of the diagnosis of B-mode ultrasound.In the diagnosis of anal atresia at middle position,the consistency of them was 0.727,which was higher than 0.651 of the diagnosis of B-mode ultrasound.In the diagnosis of anal atresia at high position,the consistency of them was 0.670,which was higher than 0.472 of the diagnosis of B-mode ultrasound.Conclusion:The effect of fistulographywith X-ray in the diagnosis of congenital anal atresia of neonateis better than that of conventional B-mode ultrasound.And it has better consistency with the diagnosis of surgery.
