ET技术评价内-中膜正常的终末期肾病患者桡动脉血管弹性 被引量:1

Echo tracking technology in evaluating vascular elasticity of end-stage renal disease with normal intima and medial membrane of radial artery

摘要 目的探讨血管回声跟踪技术(ET)评估内-中膜(IMT)正常的终末期肾病患者桡动脉血管弹性的临床应用价值。方法选取我院确诊为终末期肾病患者138例(75例IMT正常(B组),63例IMT增厚和(或)伴有斑块形成(C组)),35例健康体检者(A组)。应用常规二维超声和ET技术对受检者的桡动脉弹性功能进行检查,包括IMT、顺应性(AC)、脉搏波传导速度(PWVbeta)、僵硬度指数(Beta)、血管压力-应变弹性系数(Ep)、增大指数(AI)弹性参数,三组间各参数进行对比分析,与术中对照。结果二维超声检查:与A组比较,B组桡动脉IMT厚度无明显变化,差异无统计学意义(P>0.05),C组桡动脉IMT增厚,并且有动脉粥样硬化斑块形成,差异有统计学意义(P<0.05),B组和C组比较,差异有统计学意义(P<0.05)。ET检查:终末期肾病组(B组和C组)患者桡动脉PWVbeta、Ep、Beta高于A组,AC低于A组,差异均具有统计学意义(P<0.05)。结论ET技术可以较早、准确、客观地评估终末期肾病患者IMT正常的桡动脉血管弹性变化,为临床动静脉瘘术前血管评估提供准确的信息。 Objective To explore the application of vascular echo tracking technology in evaluating vascular elasticity of End-stage renal disease with normal intima and medial membrane of radial artery.Methods 138 patients with end-stage renal disease,including 75 patients with normal IMT(group B),63 patients with endometrial thickening and/or plaque formation(group C),and 35 healthy subjects(group A)diagnosed in our hospital were selected.Echo-tracking technology was used to check radial artery elasticity,including intima-media thickness(IMT),arterial compliance(AC),one-point pulse wave velocity(PWVbeta),stiffness parameter(Beta),pressure-strain elasticity modulus(Ep),and augmentation index(AI).The parameters of three groups were compared,analyzed,and compared with the intraoperation.Results Two-dimensional ultrasound examination:compared with group A,IMT thickness of radial artery in group B showed no significant change,and the difference was not statistically significant(P>0.05);IMT thickness of radial artery in group C increased with atherosclerotic plaque formation,and the difference was statistically significant(P<0.05).ET examination:PWVbeta,Beta and Ep of radial artery in patients with end-stage renal disease(group B and group C)were higher than those in group A,AC was lower than group A.The difference was statistical significance(P<0.05).Conclusion ET can be used to evaluate the changes of radial artery vascular elasticity with normal IMT thickness in patients with end-stage renal disease early,accurately and objectively,thereby providing accurate and valuable information for the preoperative vascular assessment of clinical arteriovenous fistula.
