摘要
全内镜特指整合了照明、摄像、灌洗系统、工作通道的硬质杆状内镜。全内镜下脊柱手术把脊柱外科手术相关的关键要素如照明、手术视野及手术工具等进行最优化组合,使手术入路损伤最小化、手术视野照明最佳化、手术视野更清晰化及手术操作更精细化[1-5]。全内镜下腰椎椎体间融合术最早由Leu等[6-7]于1996年报道,此后该技术并没有得到快速改进及推广。
Full-endoscope refers specifically to a rigid rod-shaped endoscope that integrates lighting,camera,irrigation system and working channel.Full-endoscopic spine surgery optimizes key elements related to spinal surgery including lighting,surgical vision,and surgical tools to minimize injury,maximize illumination and vision to ensure accuracy.As a minimally invasive technique of lumbar interbody fusion,indications and key techniques should follow the standards of traditional open surgery.However,individualized hybrid surgical plan based on pathological anatomy is recommended due to different surgical approaches for full-endoscopic lumbar discectomy and spinal canal decompression,various options for intervertebral fusion,preparation of bone graft beds,bone graft materials,intervertebral fusion cages,and supplemental fixation methods.Skilled full-endoscopic lumbar spinal decompression techniques as the basis,in-depth understanding of open TLIF or MIS-TLIF,strict selection of indications,individualized hybrid surgical plan are required to facilitate safe,effective clinical outcomes.
