摘要
目的:探究高频彩超应用于乳腺癌腋窝淋巴结转移早期诊断中的价值。方法:回顾性分析2017年9月-2019年9月于笔者所在医院住院治疗的乳腺癌患者98例,以术后淋巴结病理检查作为乳腺癌淋巴结转移的金标准进行分组,其中转移组56例,未转移组42例,比较两组术前腋窝淋巴结高频彩超声像特征和血流特征,评价高频彩超应用于乳腺癌腋窝淋巴结转移早期诊断中的诊断效能。结果:转移组无增厚患者占比低于非转移组,长短径之比>2患者占比高于非转移组,存在钙化斑患者占比高于非转移组,边界存在融合患者占比高于非转移组,差异均有统计学意义(P<0.05)。转移组周边型占比高于非转移组,门型占比低于非转移组,差异均有统计学意义(P<0.05)。两组淋巴结血流信号分级比较差异有统计学意义(P<0.05)。高频彩超诊断乳腺癌淋巴结转移的灵敏度为85.7%,特异度为85.7%,准确度为85.7%。结论:高频彩超能够通过声像特征和血流特征对淋巴结进行评价,应用于乳腺癌腋窝淋巴结转移早期诊断的价值较高,值得临床推广。
Objective:To explore the value of high frequency color Doppler ultrasound in the early diagnosis of axillary lymph node metastasis of breast cancer.Method:A retrospective analysis of 98 breast cancer patients who were hospitalized in our hospital from September 2017 to September 2019 was made.The pathological examination of lymph nodes after operation was used as the gold standard for breast cancer lymph node metastasis.Among them,56 cases were in the metastasis group and 42 cases were in the non metastasis group.The characteristics of high-frequency color ultrasound and blood flow of axillary lymph nodes before operation were compared between the two groups to evaluate the application of high-frequency color ultrasound in axillary lymph nodes of breast cancer.The diagnostic efficacy of early diagnosis of nodal metastasis.Result:The proportion of patients with non-thickening in metastasis group was lower than that in nonmetastasis group,the proportion of patients with long and short diameter>2 was higher than that in non-metastasis group,the proportion of patients with calcification spot was higher than that in non-metastasis group,and the proportion of patients with boundary fusion was higher than that in non-metastasis group,the differences were statistically significant(P<0.05).The proportion of peripheral type was higher in the metastasis group than that in the non metastasis group,and the proportion of portal type was lower in the non metastasis group,the differences were statistically significant(P<0.05).There was significant difference of lymph node blood flow signal classification between the two groups(P<0.05).The sensitivity,specificity and accuracy of high frequency color Doppler ultrasound in the diagnosis of breast cancer lymph node metastasis were 85.7%,85.7%and 85.7%,respectively.Conclusion:High frequency color Doppler ultrasound can evaluate the lymph nodes by the characteristics of sonography and blood flow,and it is valuable in the early diagnosis of axillary lymph node metastasis of breast cancer.
