摘要
目的探讨实时超声弹性成像及其参数对宫颈癌的诊断价值。方法选择行宫颈病理检查的女性患者90例,病理诊断为32例宫颈癌(恶性组)与宫颈癌前病变(良性组)58例。所有患者都给予实时超声弹性成像检查,记录诊断参数并判断诊断价值。结果恶性组的超声弹性成像评分显著高于良性组(P<0.05)。恶性组的Emax和Emean值都高于良性组,Emin值低于良性组,对比差异有统计学意义(P<0.05)。恶性组的间质比与胶原纤维含量显著低于良性组(P<0.05),弹力纤维含量显著高于良性组(P<0.05)。在90例患者中,实时超声弹性成像判定为恶性病灶59例,良性病灶21例,其鉴别诊断宫颈癌的敏感性为87.5%(28/32),特异性为94.8%(55/58)。结论实时超声弹性成像及其参数能有效鉴别宫颈病灶的良恶性,能为宫颈癌的早期诊断提供有意义的参考价值。
Objective To investigate the diagnostic value of real-time ultrasound elastography and its parameters in cervical cancer.Methods 90 female patients who underwent cervical pathological examination were selected that included 32 cases of cervical cancer(malignant group)and 68 cases of cervical precancerous lesions(benign group).All patients were given real-time ultrasound elastography and recorded diagnostic parameters,and were to determine diagnostic value.Results The ultrasound elastography scores of the malignant group was significantly higher than those of the benign group(P<0.05).The Emax and Emean values in the malignant group were higher than those of the benign group,and the Emin value was lower than that of the benign group,compared the difference were statistically significant(P<0.05).The interstitial ratio and collagen fiber content in the malignant group were significantly lower than that of the benign group(P<0.05),and the elastic fiber content was significantly higher than that in the benign group(P<0.05).Among 90 patients,59 cases of malignant lesions and 21 cases of benign lesions were identified by real-time ultrasound elastography.The sensitivity of differential diagnosis of cervical cancer was 87.5%(28/32),and the specificity was 94.8%(55/58).Conclusion Real-time ultrasound elastography and its parameters can effectively identify the benign and malignant cervical lesions,and provide meaningful reference for the early diagnosis of cervical cancer.
