可溶性CD40配体、脂蛋白相关磷脂酶A2和超敏C反应蛋白与急性脑梗死患者颈动脉粥样硬化斑块形成及类型的相关性 被引量:18

摘要 目的探讨血清可溶性CD40配体(sCD40L)、脂蛋白相关磷脂酶A2 (Lp-PLA2)和超敏C反应蛋白(hs-CRP)与急性脑梗死患者颈动脉粥样硬化斑块形成及类型的相关性。方法选取急性脑梗死患者270例为研究对象,所有脑梗死患者均行颈动脉血管超声检查,根据颈动脉血管超声结果将脑梗死患者分为无斑块组(60例)和有斑块组(210例)。根据斑块性质,有斑块组又分为稳定斑块组(78例)和不稳定斑块组(132例)。采用酶联免疫吸附法检测sCD40L、Lp-PLA2质量浓度,采用免疫比浊法检测hs-CRP质量浓度,比较分析上述指标结果。结果与无斑块组比较,有斑块组患者s CD40L、Lp-PLA2和hs-CRP质量浓度增高,差异有统计学意义(P <0.01);与稳定斑块组比较,不稳定斑块患者sCD40L、Lp-PLA2和hs-CRP质量浓度增高,差异有统计学意义(P <0.01)。Pearson相关分析显示,sCD40L、Lp-PLA2和hs-CRP质量浓度与颈动脉斑块的稳定性呈正相关关系(r分别为0.553、0.682、0.778,均P <0.01)。结论 sCD40L、Lp-PLA2和hs-CRP质量浓度与急性脑梗死患者颈动脉粥样硬化斑块形成及斑块类型密切相关。 Objective To investigate the association of serum soluble CD40 ligand(sCD40L), lipoprotein associated phospholipase A2(Lp-PLA2) and high sensitivity C-reactive protein(hs-CRP) with carotid atherosclerotic plaque formation and plaque types in patients with acute cerebral infarction. Methods 270 patients with acute cerebral infarction were selected as the subjects. All patients with cerebral infarction underwent carotid vascular ultrasonography. According to the results of carotid vascular ultrasound, patients with cerebral infarction were divided into a non-plaque group(60 cases) and a plaque group(210 cases). According to the nature of the plaque, the plaque group was further divided into a stable plaque group(78 cases) and an unstable plaque group(132 cases). The concentrations of sCD40L and Lp-PLA2 were detected by enzyme-linked immunosorbent assay. The concentration of hs-CRP was detected by immunoturbidimetric assay. The results of the above indicators were compared. Results Compared with the non-plaque group, the concentrations of sCD40L, Lp-PLA2 and hs-CRP were higher than those in patients in the plaque group, the difference was statistically significant(P < 0.01);Compared with the unstable plaque group, the concentrations of sCD40 L,Lp-PLA2 and hs-CRP were higher than those in patients in the stable plaque group, the difference was statistically significant(P < 0.01). Pearson correlation analysis showed that the concentrations of sCD40L, Lp-PLA2 and hs-CRP were positively correlated with carotid plaque stability(r were 0.553, 0.682, 0.778, P < 0.01).Conclusion The concentrations of sCD40L, Lp-PLA2 and hs-CRP are closely related to carotid atherosclerotic plaque formation and plaque type in patients with acute cerebral infarction.
