摘要
目的比较经皮肾镜碎石术(PCNL)与输尿管软镜碎石术(FURL)治疗肾结石(<2cm)的疗效及对免疫功能的影响.方法2017年1月至2018年2月肾结石(直径<2cm)患者100例,采用随机数表法分为PCNL组和FURL组,每组各50例.比较两组疗效及术前、术后1h、1d、3d和5d白细胞计数(WBC)、C-反应蛋白(CRP)、白细胞介素(IL)-2、干扰素(IFN)-γ、IL-6、IL-10和皮质醇(Cor)水平.结果FURL组结石一次性清除率为86.00%,总清除率为98.00%;PCNL组结石一次性清除率为82.00%,总清除率为96.00%,两组比较差异无统计学意义(P>0.05).两组手术时间比较差异无统计学意义(P>0.05),FURL组术后住院时间短于PCNL组,差异有统计学意义(P<0.05).术后1h、1d、3d和5d时,两组CRP、Cor和WBC水平均比术前高(P<0.05),且FURL组比PCNL组低(P<0.05).CRP、Cor和WBC水平在术后1h最高,随后逐渐下降.术后1h、1d、3d和5d时,两组IL-2、IFN-γ、IL-6和IL-10水平均较术前高(P<0.05),且FURL组比PCNL组低(P<0.05).IL-2、IFN-γ、IL-6和IL-10水平在术后1h最高,随后逐渐下降.结论FURL和PCNL治疗肾结石的疗效相当,但FURL对患者的免疫功能影响较小.
Objective To investigate the efficacy of percutaneous nephrolithotomy(PCNL)and ureteroscopic lithotripsy(FURL)in treating renal calculi(<2cm)and its influence on the immune function of patients.Methods One hundred patients with renal calculi(diameter<2cm)in our hospital from January 2017 and February 2018 were included in this study,and randomly divided into PCNL group and FURL group,with 50 patients in each group.The therapeutic effect of PCNL and FURL in the treatment of renal calculi were compared.The levels of leukocyte count(WBC),C-reactive protein(CRP),interleukin(IL)-2,interferon(IFN)-gamma,IL-6,IL-10 and cortisol(Coe)were compared between two groups at preoperative,postoperative 1h,postoperative 1d,3d and 5d.Results The one-time clearance rate of stone in group FURL was 86%,the total clearance rate was 98%,and the one-time clearance rate of calculi in group PCNL was 82%and the total clearance rate was 96%,there was no statistical difference between the two groups(P>0.05).There was no significant difference in the operative time between the two groups(P>0.05).The postoperative hospitalization time in group FURL was shorter than that in group PCNL(P<0.05).At postoperative 1h,1d,3d and 5d,the levels of CRP,Cor and WBC in the two groups were higher than before operation,and the FURL group was lower than the PCNL group.The levels of CRP,Cor and WBC were the highest at 1h after surgery,and then gradually decreased.At postoperative 1h,1d,3d and 5d,the levels of IL-2,IFN-,IL-6 and IL-10 in the two groups were higher than before operation,and the FURL group was lower than the PCNL group.The levels of IL-2,IFN-γ,IL-6 and IL-10 were the highest in 1h after operation,and then gradually decreased.Conclusion FURL and PCNL are effective in treating renal calculi,but FURL has little effect on the immune function of patients.
